Eustachian tube dysfunction?
Hello, doctor.
I would like to ask if the Eustachian tube only opens during swallowing and yawning.
If I continuously tense the muscles in my neck and throat, can that keep the Eustachian tube open? Are there any actions that can keep the Eustachian tube open? Thank you.
Hu Hu, 30~39 year old female. Ask Date: 2015/10/30
Dr. Zheng Jueyi reply Otolaryngology
Hello, the Eustachian tube primarily opens during swallowing and yawning.
Generally, maintaining tension in the neck and throat muscles does not allow the Eustachian tube to remain open continuously.
In fact, the Eustachian tube is closed most of the time.
A persistently open Eustachian tube can lead to Eustachian tube dysfunction, which may cause continuous echoing while speaking, resulting in increased discomfort.
Wishing you peace and health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan General Hospital, Ministry of Health and Welfare.

The Eustachian tube (ET) is a crucial structure that connects the middle ear to the nasopharynx, playing a vital role in equalizing air pressure on both sides of the eardrum and facilitating drainage of secretions from the middle ear. Understanding how the Eustachian tube functions and the mechanisms that allow it to open can help address various ear-related issues, such as pressure changes during altitude changes or ear infections.
Typically, the Eustachian tube remains closed and opens during specific actions such as swallowing, yawning, or chewing. These actions activate the muscles around the tube, particularly the tensor veli palatini and the levator veli palatini, which help to open the tube and allow air to flow in or out of the middle ear. This process is essential for maintaining equal pressure on both sides of the eardrum, which is crucial for proper hearing.
Regarding your question about whether continuous tension in the throat and neck muscles can keep the Eustachian tube open, it's important to note that while muscle tension can influence the opening of the tube, it is not a sustainable or effective method for keeping it open. Prolonged tension in the muscles can lead to discomfort or strain without providing the desired effect of keeping the Eustachian tube open. The Eustachian tube is designed to open intermittently rather than remain open continuously.
There are specific exercises and techniques that can help promote the opening of the Eustachian tube. Here are a few methods:
1. Swallowing: Regular swallowing can help open the Eustachian tube. You can try swallowing while holding your nose and closing your mouth to create a slight pressure difference.
2. Yawning: Yawning is another natural way to open the Eustachian tube. You can induce a yawn by taking a deep breath and letting it out slowly.
3. Valsalva Maneuver: This technique involves taking a deep breath, pinching your nose shut, closing your mouth, and gently blowing as if you are trying to exhale. This can help equalize pressure in the ears.
4. Toynbee Maneuver: This involves swallowing while pinching your nose shut. This technique can also help open the Eustachian tube.
5. Chewing Gum or Sucking on Candy: These actions promote saliva production and encourage swallowing, which can help keep the Eustachian tube functioning properly.
6. Neck and Jaw Exercises: Gentle stretching and movement of the neck and jaw can help relieve tension in the surrounding muscles, potentially aiding in the function of the Eustachian tube.
If you are experiencing persistent issues with your Eustachian tube, such as frequent blockages or discomfort, it may be beneficial to consult with an ear, nose, and throat (ENT) specialist. They can assess your condition and provide tailored advice or treatment options, which may include exercises, medications, or other interventions to improve Eustachian tube function.
In summary, while the Eustachian tube primarily opens during specific actions like swallowing and yawning, there are exercises and techniques that can help facilitate its function. Continuous muscle tension is not an effective method for keeping the tube open, and it is essential to use appropriate techniques to promote its proper function. If symptoms persist, seeking professional medical advice is recommended.
