Anal itching
Hello, Doctor Gao.
I have been experiencing anal itching for some time now, and it has been affecting my sleep at night.
I previously consulted a dermatologist, but the ointment they prescribed did not improve my condition.
The doctor also recommended taking medication for roundworms, but that didn't help either.
A colorectal specialist mentioned that it is not related to hemorrhoids.
I truly feel lost and am seeking guidance on which specialist I should consult.

Dear Ms.
Lin,
Regarding anorectal diseases, it is recommended that you consult a colorectal surgeon.
Thank you, and I wish you peace and good health.

Persistent anal itching, also known as pruritus ani, can be a frustrating and uncomfortable condition. Given your experience of having consulted both a dermatologist and a proctologist without finding relief, it’s understandable that you feel lost in seeking the right specialist.
In cases of persistent anal itching, the first step is to ensure that any underlying conditions are properly diagnosed. Since you have already seen a dermatologist and a colorectal surgeon (proctologist), it may be beneficial to consider consulting a gastroenterologist. Gastroenterologists specialize in the digestive system and can evaluate for conditions that may not be immediately apparent, such as inflammatory bowel disease, infections, or other gastrointestinal issues that could contribute to your symptoms.
Additionally, it might be worthwhile to consider the following potential causes of anal itching that could be addressed by a gastroenterologist or another specialist:
1. Dietary Factors: Certain foods can irritate the anal area, leading to itching. Spicy foods, caffeine, and alcohol are common culprits. A gastroenterologist can help assess your diet and suggest modifications.
2. Skin Conditions: While you have seen a dermatologist, it may be helpful to revisit them or seek a second opinion. Conditions like eczema, psoriasis, or fungal infections can cause persistent itching and may require different treatment approaches.
3. Hygiene Practices: Sometimes, improper hygiene can exacerbate anal itching. A gastroenterologist can provide guidance on proper cleaning techniques and recommend suitable products that are gentle on the skin.
4. Infections: Fungal infections, pinworms, or other parasitic infections can lead to anal itching. Although you mentioned taking medication for worms, it might be worth discussing further testing with a gastroenterologist to rule out any infections.
5. Allergic Reactions: Allergies to soaps, wipes, or laundry detergents can also cause irritation. A thorough review of your personal care products with a specialist may help identify any potential allergens.
6. Psychological Factors: Stress and anxiety can sometimes manifest as physical symptoms, including itching. If you suspect that your symptoms may be exacerbated by psychological factors, consulting a mental health professional may also be beneficial.
In summary, while you have already consulted with a dermatologist and a proctologist, I recommend seeing a gastroenterologist for a comprehensive evaluation of your symptoms. They can help identify any underlying gastrointestinal issues and provide a tailored treatment plan. Additionally, consider discussing your symptoms with your primary care physician, who can help coordinate care and refer you to the appropriate specialists.
Remember, persistent anal itching can be a multifactorial issue, and it may take time to identify the root cause and find effective treatment. Be patient with yourself during this process, and don’t hesitate to advocate for your health by seeking further opinions if necessary.
