Ear discomfort?

In recent months, I sometimes hear my own voice when speaking, and I experience intermittent tinnitus.
Additionally, when I clap my hands, I only hear the tinnitus in my left ear, while the right ear remains unaffected.
I also feel that my left ear is becoming increasingly sensitive to loud sounds.
All of these symptoms are occurring only in my left ear.
By the way, I have a history of nasal allergies, and I had otitis media in my left ear during childhood.
What should I do, and what could be the issue?

Hsu, 10~19 year old female. Ask Date: 2015/11/08

Hello: It may be due to Eustachian tube dysfunction.
If it continues to persist without improvement, it is advisable to seek medical evaluation for confirmation.
Wishing you safety and health.

More Info

Understanding ear discomfort can be quite complex, especially when symptoms like ear ringing (tinnitus), sensitivity to sound, and past medical history are involved. Based on the symptoms you've described, it seems that you may be experiencing a combination of issues related to your left ear, potentially linked to your history of middle ear infections and your current nasal allergies.

Symptoms Breakdown
1. Tinnitus: The ringing or buzzing sound in your ear, which you mentioned occurs intermittently, is known as tinnitus. This condition can be caused by various factors, including exposure to loud noises, earwax buildup, age-related hearing loss, or underlying ear conditions.

2. Sound Sensitivity: Your sensitivity to loud sounds in the left ear could indicate a condition known as hyperacusis, where normal environmental sounds are perceived as overly loud or uncomfortable. This can sometimes occur after ear infections or trauma to the ear.

3. Asymmetrical Symptoms: The fact that only your left ear is affected while the right ear remains unaffected is significant. This asymmetry can suggest localized issues in the left ear, such as fluid buildup, eustachian tube dysfunction, or even residual effects from previous infections.

4. History of Middle Ear Infections: Your past experience with middle ear infections (otitis media) could predispose you to recurrent issues. Chronic or recurrent infections can lead to changes in the ear's anatomy or function, potentially causing ongoing discomfort or sensitivity.

5. Nasal Allergies: Allergies can lead to inflammation in the nasal passages and eustachian tubes, which connect the middle ear to the back of the throat. This inflammation can affect ear pressure regulation and contribute to discomfort or tinnitus.

Recommended Actions
1. Consult an Ear, Nose, and Throat (ENT) Specialist: Given your symptoms and medical history, it would be prudent to see an ENT specialist. They can conduct a thorough examination, including a hearing test and possibly imaging studies, to assess the condition of your ear and eustachian tubes.

2. Hearing Evaluation: A comprehensive audiological evaluation can help determine if there is any hearing loss associated with your symptoms. This can guide further management.

3. Management of Allergies: Since you have a history of nasal allergies, managing these through antihistamines or nasal corticosteroids may help reduce inflammation and improve eustachian tube function, potentially alleviating some of your ear symptoms.

4. Tinnitus Management: If tinnitus is persistent, cognitive behavioral therapy (CBT) or sound therapy may be beneficial. These approaches help individuals manage their perception of tinnitus and reduce its impact on daily life.

5. Avoid Loud Noises: Protect your ears from loud sounds, as exposure can exacerbate tinnitus and sensitivity. Use earplugs in noisy environments.

6. Monitor Symptoms: Keep track of your symptoms, noting any changes or triggers. This information can be valuable for your healthcare provider in determining the best course of action.

Conclusion
In summary, your symptoms of ear discomfort, tinnitus, and sound sensitivity in the left ear, combined with your history of middle ear infections and nasal allergies, suggest a multifactorial issue that warrants further evaluation. An ENT specialist can provide a comprehensive assessment and tailored treatment plan to address your concerns effectively. Remember, early intervention is key in managing ear-related issues, especially for someone with aspirations of becoming a pilot, as ear health is crucial for flying.
