Is it normal to experience some pain when biting down after completing root canal treatment?
Hello, I completed my root canal treatment on November 7th.
As the doctor mentioned, it did hurt a bit right after the procedure, but the pain subsided within 1 to 2 days.
By November 8th, that tooth was indeed pain-free.
However, today while eating something hard, I bit down on that tooth and experienced a sudden sharp pain! But when I’m not chewing or eating, there’s no pain.
Is this normal?

Dr. Zeng Guanghui reply Dentistry
After root canal treatment, discomfort while eating for 1-2 days may occur, which could be due to inflammation of the periapical tissues.
This should gradually improve within 2-3 days.
If the discomfort persists for more than three days, it is advisable to return to the dentist for further evaluation to determine if there is still an infection at the root apex.

Experiencing pain when biting down after a root canal treatment can be concerning, and it is important to understand the factors that may contribute to this discomfort. After a root canal procedure, it is common for patients to experience some level of discomfort or pain, especially in the days following the treatment. This is primarily due to the inflammation and healing process that occurs after the removal of the dental pulp and the cleaning of the root canals.
In your case, since you mentioned that the pain subsided after the initial days but then returned when biting down on hard food, there are several possible explanations for this phenomenon:
1. Inflammation and Healing: After a root canal, the surrounding tissues may still be inflamed. This inflammation can cause sensitivity, especially when pressure is applied, such as when biting into harder foods. It is not unusual for this discomfort to persist for a few weeks as the area heals.
2. Temporary Filling: If a temporary filling was placed after the root canal, it may not provide the same level of support as a permanent restoration. This can lead to discomfort when biting down, especially on harder foods. If the temporary filling is compromised or not properly sealed, it may also allow for bacteria to enter, leading to further irritation.
3. High Bite: Sometimes, after a root canal treatment, the bite may be slightly off due to the shape of the temporary filling or the way the tooth was restored. If your bite is not aligned properly, it can cause discomfort when chewing. It is advisable to have your dentist check your bite to ensure that everything is aligned correctly.
4. Underlying Issues: Although rare, there may be underlying issues such as residual infection or inflammation at the root tip (apical periodontitis) that could cause pain when biting. If the root canal was particularly complicated, or if the tooth had a complex canal system, it may not have been completely cleaned out, leading to ongoing discomfort.
5. Jaw and Muscle Tension: Sometimes, pain can also arise from muscle tension in the jaw or issues with the temporomandibular joint (TMJ). If you are unconsciously clenching or grinding your teeth, this could lead to pain when biting down.
Given these possibilities, it is essential to communicate with your dentist about the pain you are experiencing. They may recommend a follow-up appointment to assess the tooth and ensure that everything is healing properly. They can also check your bite and make any necessary adjustments to alleviate discomfort.
In summary, while some discomfort after a root canal is normal, experiencing pain specifically when biting down on hard foods warrants further investigation. It is crucial to address these concerns with your dental professional to ensure proper healing and to rule out any complications.
