Pain upon palpation below the left ear?

Tonight, I suddenly noticed that there is a significant pain when I apply pressure to the area just below the angle of the bone on the left side of my jaw, about a finger's width down.
The pain radiates upwards towards my left eardrum, and when I press the same spot on my right ear, I feel no discomfort at all.
Since I don't usually touch this area, I'm not sure how long the pain has been present.
I would like to ask the doctor what could be the possible cause of this condition.
Thank you for your response.

C, 20~29 year old female. Ask Date: 2015/11/11

Dr. Zheng Jueyi reply Otolaryngology

Hello, if there are no obvious lumps in the neck and pain occurs only with significant pressure, it is mostly a benign manifestation.
You can observe it for now, but if you still feel uneasy, it is advisable to see a doctor for an examination.
Wishing you peace and good health.
Sincerely, Dr.
Jue-Yi Cheng, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

Pain under the left ear can be concerning, especially when it is accompanied by tenderness and discomfort that radiates towards the ear itself. There are several potential causes for this type of pain, and understanding them can help you determine the best course of action.

1. Temporomandibular Joint (TMJ) Disorders: One of the most common causes of pain in the area under the ear is related to the temporomandibular joint, which connects the jaw to the skull. TMJ disorders can result from teeth grinding, jaw clenching, or misalignment of the jaw. Symptoms may include pain when chewing, clicking sounds when moving the jaw, and tenderness in the jaw or ear area. If you notice that the pain worsens with jaw movement or is associated with headaches, TMJ dysfunction could be a likely cause.

2. Ear Infections: While you mentioned that the pain is under the ear, it is important to consider that infections in the middle ear (otitis media) can cause referred pain to the area beneath the ear. This is particularly relevant if you have had recent upper respiratory infections or allergies, which can lead to fluid buildup and infection in the ear.

3. Lymphadenopathy: The area under the ear contains lymph nodes that can become swollen due to infections or other inflammatory processes. If you have had a recent cold, throat infection, or dental issues, the lymph nodes may react by swelling, leading to tenderness and pain.

4. Dental Issues: Problems with the teeth, such as cavities or abscesses, can cause pain that radiates to the ear area. If you have any dental issues or have recently had dental work done, this could be a contributing factor.

5. Cervical Spine Issues: Sometimes, pain under the ear can be referred from issues in the cervical spine, such as muscle strain or nerve irritation. If you have been experiencing neck pain or stiffness, this could be a factor.

6. Other Causes: Other less common causes could include conditions such as mastoiditis (infection of the mastoid bone), salivary gland infections, or even referred pain from conditions affecting the throat or esophagus.

When to Seek Help
It is advisable to seek medical attention if:
- The pain persists for more than a few days without improvement.

- You experience additional symptoms such as fever, swelling, or redness in the area.

- There is a noticeable change in hearing or if you experience dizziness.

- You have difficulty opening your mouth or chewing.

- The pain is severe and affects your daily activities.

Conclusion
In summary, pain under the left ear can arise from various causes, including TMJ disorders, ear infections, lymph node swelling, dental issues, or cervical spine problems. Given the complexity of the ear and surrounding structures, it is essential to monitor your symptoms closely. If the pain persists or worsens, consulting with a healthcare professional, particularly an ear, nose, and throat (ENT) specialist or a dentist, would be prudent for a thorough evaluation and appropriate treatment. Early intervention can often lead to better outcomes and relief from discomfort.
