Nipple discharge with blood?

I am 15 years old, and I have white, pimple-like discharge from my left nipple that only appears when squeezed.
Once, after squeezing, there was blood.
The left nipple feels harder than the right, and there is a sensation of a breast lump.

Ying, 10~19 year old female. Ask Date: 2015/11/14

Dr. Zhou Jiazhen reply Breast and Thyroid

Dear Ying: 1.
There is discharge from all milk ducts; when expressing, do not apply too much pressure, as it may sometimes cause bleeding.
If bleeding occurs frequently, it is advisable to follow up at the outpatient clinic.
2.
The skin of the nipple may also have skin cysts and could have discharge.
If there is an increase in size or symptoms (pain, itching, etc.), it is recommended to follow up at the outpatient clinic.

When it comes to experiencing unusual discharge from the nipple, especially in a teenager, it can be concerning and may raise questions about underlying health issues. In your case, the presence of white discharge resembling a pimple, along with occasional blood and a feeling of hardness in the left breast, warrants careful consideration.

Firstly, it's important to understand that breast tissue undergoes various changes during puberty due to hormonal fluctuations. These changes can lead to the development of benign conditions such as fibrocystic breast changes, which may cause lumps, tenderness, and discharge. The white discharge you describe could be related to these hormonal changes, particularly if it is not accompanied by other alarming symptoms.

However, the presence of blood in the discharge and the feeling of hardness in one breast compared to the other should not be ignored. While it is possible that these symptoms are benign, they can also indicate conditions that require further evaluation. Some potential causes of nipple discharge include:
1. Hormonal Changes: As mentioned, hormonal fluctuations during puberty can lead to various breast changes, including discharge.

2. Infections: Conditions such as mastitis or other infections can cause discharge and may be accompanied by redness, swelling, or pain.

3. Duct Ectasia: This occurs when a milk duct beneath the nipple dilates, fills with fluid, and thickens, which can lead to discharge and sometimes blood.

4. Fibroadenomas: These are benign tumors that can develop in breast tissue, often felt as lumps. They are usually painless but can cause changes in the breast.

5. Intrductal Papilloma: A small, benign tumor that forms in a milk duct can cause discharge, sometimes with blood.

6. Rare Conditions: While less common, conditions such as breast cancer can also present with unusual discharge, especially if there are other concerning symptoms.

Given your symptoms, it is crucial to consult a healthcare provider, preferably a pediatrician or a specialist in adolescent medicine. They may recommend a physical examination and possibly imaging studies, such as an ultrasound, to assess the breast tissue and any lumps or abnormalities. Blood tests may also be conducted to rule out infections or hormonal imbalances.

In summary, while hormonal changes during puberty can lead to benign breast conditions, the presence of blood and a noticeable difference in breast texture should prompt a medical evaluation. Early assessment and diagnosis are key to addressing any potential issues and ensuring peace of mind. Remember, it's always better to err on the side of caution when it comes to health concerns, especially those involving changes in the body.
