Frequent bowel movements, abdominal distension with a feeling of pressure?
Hello Doctor, in recent days I have been experiencing frequent discharge of clear intestinal fluid, and sometimes I also pass yellowish fluid that resembles stool.
Additionally, I have been feeling bloating in my right abdomen, to the extent that my belly visibly protrudes when bloated, accompanied by pain and a sensation of extreme pressure as if my intestines are about to burst.
This sensation lasts for a few seconds and is followed by a loud gurgling noise.
I also experience similar sensations in my lower left abdomen.
My bowel movements are infrequent, consisting of thin stools about the length of two finger joints.
Thank you.

Dr. Cai Anshun reply Gastroenterology and Hepatology
Hello: It may be due to retained stool or intestinal gas.
Please drink plenty of water and reduce the intake of fried or spicy foods.
You can perform clockwise abdominal massage around the navel to help stimulate bowel movement.
If you feel the urge to pass gas or burp, do not hold it in.
If the situation persists without improvement, you may consult a gastroenterologist for a proper evaluation.

More Info
Frequent clear bowel movements accompanied by severe abdominal distension can be indicative of several gastrointestinal issues. Based on your description, it seems that you are experiencing symptoms that may be related to a condition known as Irritable Bowel Syndrome (IBS), particularly the subtype characterized by diarrhea (IBS-D). This condition is often associated with abdominal pain, bloating, and changes in bowel habits, including frequent bowel movements that may be loose or watery.
The clear bowel movements you mentioned could be a result of increased intestinal motility, where the intestines are contracting more frequently than normal. This can lead to the expulsion of liquid stool or mucus, which may appear clear or yellowish. The presence of abdominal distension and the sensation of your abdomen feeling like it is going to "burst" can be attributed to the accumulation of gas in the intestines, which is common in IBS. The loud gurgling sounds you hear, known as borborygmi, are also a result of this increased intestinal activity and gas movement.
Your symptoms of right-sided abdominal pain and distension, along with the occasional left-sided discomfort, suggest that there may be localized irritation or inflammation in the intestines. This could be due to dietary factors, stress, or other underlying gastrointestinal conditions. It is important to consider your diet, as certain foods can exacerbate IBS symptoms. Common triggers include high-fat foods, dairy products, caffeine, and certain carbohydrates known as FODMAPs (fermentable oligosaccharides, disaccharides, monosaccharides, and polyols).
In addition to dietary factors, stress and anxiety can significantly impact gut function and may lead to increased symptoms of IBS. The gut-brain connection is well established, and emotional stress can lead to changes in gut motility and sensitivity.
Given your symptoms, it is advisable to keep a food diary to identify any potential triggers and to consider dietary modifications. A low-FODMAP diet has been shown to be effective for many individuals with IBS. Additionally, incorporating regular physical activity and stress management techniques, such as mindfulness or yoga, may help alleviate some of your symptoms.
If your symptoms persist or worsen, it is crucial to seek medical attention. A healthcare provider may recommend further evaluation, such as blood tests, stool tests, or imaging studies, to rule out other conditions such as infections, inflammatory bowel disease (IBD), or structural abnormalities in the gastrointestinal tract.
In summary, your symptoms of frequent clear bowel movements, abdominal distension, and pain may be indicative of IBS or another gastrointestinal issue. Monitoring your diet, managing stress, and consulting with a healthcare professional for a thorough evaluation and personalized treatment plan are essential steps to address your concerns effectively.
