Strange sounds in the ear?

Dr.
Hsu, recently every time I yawn or swallow, I hear a few clicking sounds in my right ear (this has been happening for over a week).
However, there is no pain, and it doesn't feel like water is trapped inside, nor does it feel like the pressure sensation associated with tinnitus.
What could be happening with my ear? Thank you.

lcy, 20~29 year old female. Ask Date: 2002/07/02

Hello: In response to your inquiry, the possible scenarios are as follows: First, the sound you hear could be due to the opening of the Eustachian tube, which is particularly noticeable when yawning, swallowing, or blowing air.
Second, there may be some earwax in the external auditory canal.
Although your hearing has not worsened, it is advisable to visit a clinic for a thorough examination for peace of mind.

Unusual sounds in the ear, such as the "clicking" or "popping" noises you describe when yawning or swallowing, can be attributed to several benign conditions, most commonly related to the Eustachian tube function. The Eustachian tube connects the middle ear to the back of the throat and helps equalize pressure in the ear. When you yawn, swallow, or even chew, this tube opens to allow air to flow into the middle ear, which can create a sound if there is any pressure change or if the tube is not functioning optimally.

Here are some potential explanations for the sounds you're experiencing:
1. Eustachian Tube Dysfunction: This is the most likely cause of the clicking sounds. If the Eustachian tube does not open properly, it can lead to pressure changes in the middle ear, resulting in sounds when you yawn or swallow. This condition can be exacerbated by allergies, colds, or sinus infections, which can cause inflammation and blockage of the Eustachian tube.

2. Middle Ear Pressure Changes: When you yawn, the act can cause a rapid change in pressure within the middle ear. If there is a slight imbalance, it can create a popping or clicking sound. This is usually harmless and resolves on its own.

3. Earwax Buildup: Sometimes, a buildup of earwax can cause unusual sounds in the ear. If earwax is pressing against the eardrum, it can create sounds when you move your jaw or yawn. However, this typically comes with other symptoms, such as a feeling of fullness in the ear or hearing loss.

4. Muscle Spasms: In rare cases, spasms of the muscles in the middle ear can produce clicking sounds. This is known as myoclonus of the middle ear and is usually not a cause for concern.

5. Tinnitus: While you mentioned that it is not tinnitus, it is worth noting that some people experience sounds in their ears that are not related to external stimuli. Tinnitus can manifest in various ways, including clicking, ringing, or buzzing sounds.

Given that you have not experienced any pain or discomfort, and the sounds occur only during yawning or swallowing, it is likely a benign issue related to Eustachian tube function. However, if the sounds persist, worsen, or are accompanied by other symptoms such as hearing loss, dizziness, or pain, it would be advisable to consult an ear, nose, and throat (ENT) specialist for a thorough examination.

In the meantime, you can try some home remedies to help with Eustachian tube function, such as:
- Yawning or Swallowing: Continue to yawn or swallow frequently, especially during altitude changes (like flying or driving in the mountains) to help equalize ear pressure.

- Nasal Decongestants: If you have allergies or a cold, using a decongestant may help reduce inflammation and improve Eustachian tube function.

- Valsalva Maneuver: Gently pinch your nose, close your mouth, and try to exhale. This can help open the Eustachian tube and equalize pressure.

In conclusion, while the sounds you are experiencing are likely benign and related to Eustachian tube function, monitoring your symptoms and seeking medical advice if they change or worsen is always a prudent approach.
