Pain in the lower left second molar?

Hello Doctor, I had surgery to remove my lower left impacted wisdom tooth on November 18, and my recovery has been good.
I had my stitches removed on November 25! However, for the past two days, I have been feeling swelling and pain in my lower left second molar, which has undergone root canal treatment.
I'm not sure what to do about this! Thank you, Doctor!

The above situation may indicate that this molar has a periodontal infection.
It is recommended to consult a dentist for further diagnosis and treatment to determine if there are any pathological changes in the gingiva surrounding the molar.

After undergoing surgery to extract a lower left wisdom tooth, it is not uncommon to experience some pain and swelling in the surrounding areas, including the adjacent second molar. The timeline you provided indicates that you had the extraction on November 18 and had your stitches removed on November 25. The onset of pain and swelling in the second molar a few days after the stitch removal could be attributed to several factors.

First, it is important to consider that the extraction site can be sensitive for several days to weeks post-surgery. The healing process involves inflammation, which can lead to swelling and discomfort in the surrounding teeth and gums. If the second molar had previously undergone root canal treatment, it may be more susceptible to irritation or infection, especially if the extraction site is still healing.

Another potential cause of pain and swelling in the second molar could be related to the surgical procedure itself. During the extraction of the wisdom tooth, there may have been some trauma to the adjacent teeth or tissues, leading to temporary discomfort. Additionally, if there was any infection present prior to the extraction, it could have been exacerbated by the surgery, resulting in pain and swelling.

Here are some steps you can take to manage the pain and swelling:
1. Cold Compress: Applying a cold compress to the outside of your cheek near the extraction site can help reduce swelling and numb the area, providing some relief from pain.

2. Pain Management: Over-the-counter pain medications, such as ibuprofen or acetaminophen, can be effective in managing discomfort. Be sure to follow the recommended dosage on the packaging or as advised by your dentist.

3. Salt Water Rinse: After the first 24 hours post-surgery, gently rinsing your mouth with warm salt water can help keep the extraction site clean and may reduce inflammation.

4. Avoid Irritants: Stay away from smoking, alcohol, and spicy or hard foods that could irritate the extraction site and the adjacent tooth.

5. Monitor Symptoms: Keep an eye on your symptoms. If the swelling and pain persist or worsen, or if you develop a fever, it could indicate an infection that requires medical attention.

6. Follow-Up Appointment: It is crucial to follow up with your dentist or oral surgeon if the pain and swelling do not improve within a few days. They may need to evaluate the area for any signs of infection or other complications.

In summary, while some discomfort and swelling after a wisdom tooth extraction is normal, persistent pain in the adjacent second molar, especially one that has undergone root canal treatment, should be evaluated by a dental professional. They can provide a thorough examination and determine if any further treatment is necessary, such as antibiotics for an infection or additional care for the second molar. Always prioritize communication with your healthcare provider regarding any concerns you may have during your recovery.
