Is floaters a complication of cataracts?

I have not experienced floaters for 65 years.
I was diagnosed with cataracts 2-3 months ago, and I had double vision in my left eye, which led me to seek surgical treatment.
The right eye did not show symptoms of double vision.
Due to the severity of the cataract in the left eye, I underwent successful surgery on that eye, and I did not develop floaters afterward.
The cataract in my right eye was less severe, so I sought another doctor for surgery to ensure the best outcome.
However, immediately after the surgery on my right eye, I noticed numerous small dark spots, indicating the presence of floaters.
Dr.
Wang, is it possible that the floaters in my right eye, which were not present before, were caused by the surgical technique of the new doctor? Thank you, Dr.
Wang, for your guidance.

Dr. Wang Zhangji reply Ophthalmology

After cataract surgery, varying degrees of floaters are commonly observed.
This is due to the increased light exposure post-surgery, which makes it easier to notice the opacities in the vitreous body.
Most cases are not serious, but it is advisable to have an eye doctor examine the retina.

More Info

Floaters, or "floaters in the eye," are small spots or lines that appear in a person's field of vision, often described as cobwebs or shadows. They are typically caused by changes in the vitreous gel that fills the eye, which can become more liquid with age, leading to the formation of these floaters.
In the context of cataract surgery, the occurrence of floaters post-operatively can be a concern for patients. While cataract surgery itself does not directly cause floaters, the surgery can sometimes lead to changes in the vitreous gel, which may result in the sudden appearance of floaters. This is particularly relevant for patients who may have had a pre-existing condition that was not noticeable before the surgery.

In your case, it is important to understand that the development of floaters after cataract surgery can happen for several reasons:
1. Vitreous Changes: As mentioned, the surgery can alter the vitreous gel's structure, leading to the formation of floaters. This is a common occurrence and is not necessarily indicative of a surgical error.

2. Pre-existing Conditions: If you had a healthy vitreous before surgery, the sudden appearance of floaters could be coincidental and not directly related to the surgical procedure.
3. Surgical Technique: While it is unlikely that a change in surgeon would directly cause floaters, different surgical techniques and approaches can influence the vitreous. Some surgeons may manipulate the vitreous more than others during cataract surgery, which could potentially lead to floaters.

4. Post-operative Inflammation: Inflammation following surgery can also lead to changes in the vitreous, contributing to the perception of floaters.

5. Retinal Issues: In some cases, the appearance of new floaters can be associated with more serious conditions, such as retinal tears or detachment. It is crucial to monitor any new symptoms, especially if they are accompanied by flashes of light or a sudden increase in floaters.

Given your history of cataract surgery and the sudden onset of floaters in your right eye, it is advisable to consult with your ophthalmologist. They can perform a thorough examination to rule out any complications, such as retinal issues, and provide guidance on managing the floaters.
In summary, while cataract surgery can lead to the appearance of floaters, it is not a direct side effect of the procedure itself. The development of floaters can be influenced by various factors, including pre-existing conditions and changes in the vitreous gel. It is essential to communicate any concerns with your healthcare provider to ensure proper evaluation and care.
