Pregnancy test strip
Hello Dr.
Lu, this morning I used a pregnancy test strip and there was only one clear line, with no second line appearing.
If there are signs of pregnancy, is it possible that I might not test positive even 24 days after intercourse? Can I rule out the possibility of pregnancy? Thank you.
Niming, 20~29 year old female. Ask Date: 2015/11/29
Dr. Lü Lizheng reply Obstetrics and Gynecology
After sexual intercourse, pregnancy can typically be detected 18 days later.
A negative result can rule out the possibility of pregnancy.
Sincerely, Dr.

When it comes to understanding pregnancy tests, particularly those that yield a single line, it’s essential to grasp the science behind these tests and the timing of when they are taken. A pregnancy test typically detects the presence of human chorionic gonadotropin (hCG), a hormone produced shortly after a fertilized egg attaches to the uterus. This hormone can usually be detected in urine about 14 days after conception, which corresponds to the time of a missed period for most women.
In your case, you mentioned using a pregnancy test 24 days after intercourse and receiving a negative result (one line). This could indicate that you are not pregnant, especially if your menstrual cycle is regular. However, there are several factors to consider that could affect the accuracy of the test:
1. Timing of the Test: If you took the test too early, it’s possible that hCG levels were not high enough to be detected. While some tests claim to be sensitive enough to detect pregnancy a few days before a missed period, the most reliable results are obtained after a missed period.
2. Test Sensitivity: Different pregnancy tests have varying sensitivities to hCG. Some tests can detect lower levels of the hormone than others. If you used a less sensitive test, it might not have detected pregnancy even if it were present.
3. Dilution of Urine: The concentration of hCG in urine can vary based on hydration levels. Taking the test with diluted urine (for example, after drinking a lot of fluids) can lead to a false negative. It is generally recommended to take the test with first-morning urine, which is usually more concentrated.
4. Chemical Pregnancy: In some cases, a fertilized egg may implant but not develop properly, leading to a very early miscarriage. This can result in a situation where you might experience early pregnancy symptoms but test negative.
5. Other Factors: Hormonal imbalances, certain medications, or medical conditions can also influence the results of a pregnancy test.
Given your situation, if you are experiencing signs of pregnancy but have received a negative test result, it may be advisable to wait a few days and retest, preferably using first-morning urine. If you continue to experience symptoms or if your period does not arrive, consulting with a healthcare provider for a blood test or further evaluation would be a prudent step. Blood tests can detect lower levels of hCG and provide a more definitive answer regarding pregnancy.
In summary, while a single line on a pregnancy test typically indicates that you are not pregnant, various factors can influence this result. If you have concerns or persistent symptoms, seeking medical advice is always a good approach to ensure your health and peace of mind.
