Persistent throat pain?

I have been experiencing recurrent throat inflammation for a long time.
It gets better for a while and then flares up again.
Sometimes, after chatting for a bit, I feel pain, and I can't even sing; even singing softly is impossible.
In the winter, I wake up with unbearable throat pain, which is truly severe.
I have to drink several sips of water upon waking.
Even in the summer, I often feel a dull ache in my throat, which can persist throughout the day.
After singing just two songs, my throat becomes very painful, and when I look in the mirror, I can see five white spots at the back of my throat.
It takes two to three days for those white spots to disappear.
I have seen a doctor, who mentioned that my throat might be more sensitive, but there was no definitive conclusion.
I remember not having this issue in the past; now it fluctuates between good and bad.
I might feel fine today, but tomorrow speaking can cause pain, and my tonsils are not inflamed...
I really want to return to the time when my throat didn't hurt, but I can't find the underlying cause.

Hello: There are many factors that can cause chronic throat inflammation, including gastroesophageal reflux, postnasal drip, smoking, air pollution, and spicy foods.
It is recommended to maintain a regular routine and observe if any related factors are contributing to the chronic inflammation.
Wishing you safety and health.
Sincerely, Dr.

Chronic sore throat can be a frustrating and debilitating condition, especially when it recurs frequently and affects daily activities such as speaking and singing. Based on your description, it seems that you have been experiencing recurrent throat inflammation, which can be caused by a variety of factors. Understanding these causes is crucial for effective management and relief.

One of the most common causes of chronic sore throat is postnasal drip, which occurs when mucus from the nasal passages drips down the back of the throat. This can lead to irritation and inflammation, especially if you have allergies or sinus issues. Seasonal allergies, in particular, can exacerbate this condition, leading to a cycle of throat discomfort. Additionally, environmental factors such as dry air, especially during winter months, can contribute to throat irritation.

Another potential cause of your symptoms could be gastroesophageal reflux disease (GERD). In this condition, stomach acid flows back into the esophagus, which can irritate the throat and lead to a sore throat. Symptoms of GERD may not always include heartburn, so it’s important to consider this possibility if you experience throat pain, especially after eating or lying down.

Viral infections, such as the common cold or flu, can also lead to recurrent sore throats. While you mentioned that your tonsils are not inflamed, viral infections can still cause throat discomfort without significant swelling of the tonsils. In some cases, you may also experience white spots in the throat, which could indicate a viral infection or even a mild case of strep throat, even if the classic symptoms are not present.

Chronic throat irritation can also be linked to lifestyle factors. For instance, excessive talking, singing, or shouting can strain the vocal cords and lead to throat pain. If you frequently engage in activities that require vocal strain, it may be beneficial to practice vocal rest and hydration. Additionally, smoking or exposure to secondhand smoke can irritate the throat and lead to chronic discomfort.

Given your symptoms, it would be advisable to consult with an ear, nose, and throat (ENT) specialist. They can perform a thorough examination, which may include a laryngoscopy to visualize the throat and vocal cords. This examination can help rule out any structural issues or chronic conditions that may be contributing to your symptoms.

In the meantime, there are several self-care strategies you can implement to alleviate your sore throat. Staying well-hydrated is crucial; drinking plenty of water can help soothe irritation. Gargling with warm salt water may also provide temporary relief. Additionally, using a humidifier in your home can help maintain moisture in the air, which can be beneficial, especially during dry winter months.

If allergies are suspected, over-the-counter antihistamines may help reduce postnasal drip and throat irritation. If GERD is a concern, lifestyle modifications such as avoiding spicy or acidic foods, eating smaller meals, and not lying down immediately after eating can be beneficial.

In conclusion, chronic sore throat can arise from various causes, including postnasal drip, GERD, viral infections, and lifestyle factors. A thorough evaluation by a healthcare professional is essential to identify the underlying cause and develop an appropriate treatment plan. In the meantime, implementing self-care strategies can help manage your symptoms and improve your quality of life.
