Chest Wall Itching: When to Consult an Orthopedic Specialist - Orthopedics

Chest wall pruritus

Hello, Doctor: I have a very sensitive itching sensation on the right side of my chest, specifically along the lower edge of my breast and sternum.
(It's an internal itch.) I have seen a pulmonologist and had two X-rays taken (one lateral view), and the doctor said my lungs are fine.
I also consulted a breast surgeon who performed an ultrasound.
I would like to know if this could be an orthopedic issue or if I need to see another specialist.
I'm quite worried.
Thank you.
Ms.

Itching of the sternum may not be closely related to orthopedic issues.
If there is a problem at the junction of the ribs and the sternum, it typically presents with swelling or pain.
Isolated itching may warrant consideration of other issues, such as a dermatological condition.

Hello Ms. Lin,
Thank you for reaching out with your concerns regarding the itching sensation in your right chest area, particularly around the bone and under the breast. It’s understandable to feel anxious when experiencing unusual symptoms, especially when they involve sensitive areas of the body.

Based on your description, it seems you have already taken significant steps by consulting with a pulmonologist and a breast surgeon, both of whom have ruled out issues related to the lungs and breast tissue. This is a positive sign, as it helps narrow down potential causes for your symptoms.

The itching sensation you are experiencing could be related to several factors, including dermatological issues, nerve irritation, or even musculoskeletal problems. Here are some considerations:
1. Dermatological Causes: Sometimes, skin conditions such as eczema, psoriasis, or contact dermatitis can cause localized itching. Even if there are no visible rashes, underlying skin conditions can still lead to sensations of itching.

2. Nerve Irritation: The sensation of itching can also arise from nerve irritation or damage. Conditions such as intercostal neuralgia, which involves irritation of the nerves that run between the ribs, could lead to sensations of itching or discomfort in the chest area.

3. Musculoskeletal Issues: Since you mentioned that the itching occurs when you press on the area, it could be related to musculoskeletal issues. Conditions such as costochondritis (inflammation of the cartilage connecting the ribs to the sternum) or muscle strain could potentially cause discomfort and unusual sensations in that region.

4. Referred Pain: Sometimes, issues in the spine or other areas can lead to referred sensations in the chest. For example, a herniated disc in the thoracic spine could potentially irritate nerves that lead to sensations in the chest wall.

Given that you are experiencing a persistent symptom, it would be prudent to consult with an orthopedic specialist, particularly one who has experience with thoracic conditions. They can evaluate your musculoskeletal system, including your ribs, sternum, and surrounding soft tissues, to determine if there is any underlying issue that may be contributing to your symptoms.

Additionally, if the orthopedic specialist does not find any issues, they may refer you to a neurologist or a dermatologist for further evaluation. It’s essential to keep an open line of communication with your healthcare providers and express your concerns thoroughly, as this will help them guide you to the appropriate next steps.

In the meantime, consider keeping a symptom diary to track when the itching occurs, any activities that may exacerbate it, and any other associated symptoms. This information can be valuable for your healthcare provider in diagnosing the issue.

Lastly, while it’s natural to feel worried about your health, try to focus on the steps you are taking to seek answers. Early intervention and thorough evaluation are key to addressing any potential issues effectively.

Wishing you the best in your health journey, and I hope you find relief soon.

Best regards.
