Regarding the treatment of floaters?

Dr.
Wang, I have some questions regarding the treatment of floaters.

1.
I heard that floaters can be treated with lutein or using YAG laser therapy.
There are also claims online that there are specialized clinics for treating floaters in the U.S.
and Hong Kong.
Are there any such clinics in Taiwan?

2.
If YAG laser therapy can be used for treatment, why do doctors often advise patients to just learn to live with them?
3.
Regarding floaters, you mentioned last time that the prevalence in individuals aged 20-30 is between 30-76% according to various statistics.
Does this mean treatment is necessary or not?
4.
I read online that the appearance of floaters is the beginning of eye degeneration, which could potentially lead to cataracts and retinal detachment.
Is this true?

Dr. Wang Zhishun reply Ophthalmology

Hello, only pathological floaters require treatment, and this type of treatment should be available at many medical institutions.
As for non-pathological floaters, it is recommended to coexist with them.
Simple floaters are related to issues with the vitreous body, while pathological floaters can lead to cataracts and retinal detachment.

More Info

Floaters, or "flying mosquitoes," are small specks or threads that drift through your field of vision. They are typically harmless and are caused by tiny pieces of gel or cells inside the vitreous, the clear gel-like substance that fills the eye. As we age, the vitreous can shrink and become more liquid, leading to the formation of these floaters. While many people experience floaters, especially as they get older, there are several myths and facts surrounding their treatment and implications.

1. Treatment Options: There are indeed various treatment options for floaters, including dietary supplements like lutein, which is believed to support eye health, and laser treatments such as YAG laser vitreolysis. However, the effectiveness of these treatments can vary, and they are not universally recommended. In Taiwan, as in many other countries, there may not be specialized clinics solely dedicated to treating floaters, but ophthalmologists can provide guidance and treatment options.

2. Why "Live with It"?: Many doctors advise patients to "live with" floaters because, in most cases, they are benign and do not pose a significant risk to vision. The YAG laser treatment can be effective for some patients, but it also carries risks, such as retinal detachment or bleeding. Therefore, the conservative approach of monitoring floaters is often preferred unless they significantly impair vision or are associated with other eye problems.

3. Prevalence and Need for Treatment: The prevalence of floaters is indeed high, with studies suggesting that 30-76% of people may experience them at some point in their lives. However, the need for treatment is subjective. Many individuals adapt to floaters over time, and they may become less noticeable. Treatment is generally reserved for those whose floaters significantly interfere with daily activities or quality of life.

4. Association with Eye Conditions: It is true that the appearance of floaters can sometimes be associated with age-related changes in the eye, such as the development of cataracts or retinal detachment. While floaters themselves are not a direct cause of these conditions, they can be a symptom of underlying issues. For instance, a sudden increase in floaters, especially if accompanied by flashes of light or a shadow in the peripheral vision, should prompt immediate evaluation by an eye care professional, as these could indicate retinal detachment.

In summary, while floaters are common and usually harmless, understanding their nature and the available treatment options is essential. If you are experiencing bothersome floaters, it is advisable to consult with an ophthalmologist who can assess your specific situation and recommend appropriate management strategies. Regular eye examinations are also crucial, especially as you age, to monitor for any changes in your eye health that could require attention.
