White spots on the glans?

Hello, Dr.
Hsu.
I would like to ask a question.
Previously, I noticed a red area on my glans, but today it has a slight stinging sensation with many white spots and a bit of fluid that feels like tissue fluid.
Could this be caused by inflammation of the glans? I am unsure, so I have been cleaning it with MILK body wash.
Thank you for your help, Dr.
Hsu.

Understanding the presence of white spots on the glans (the head of the penis) can be quite concerning, and it’s important to approach this issue with a clear understanding of potential causes and appropriate treatments. Based on your description, it seems that you are experiencing symptoms that could be indicative of a few different conditions, including inflammation, infection, or even a dermatological issue.

Firstly, the redness and pain you are experiencing, along with the presence of white spots and fluid, could suggest a condition known as balanitis. Balanitis is an inflammation of the glans, which can be caused by a variety of factors including poor hygiene, irritants (such as soaps or body washes), infections (both fungal and bacterial), or sexually transmitted infections (STIs). The use of a harsh soap like a milk-based body wash may irritate the sensitive skin of the glans, leading to inflammation and discomfort.

In terms of the white spots, they could be due to several reasons:
1. Pearly Penile Papules: These are small, harmless bumps that are often mistaken for a disease. They are normal anatomical variations and do not require treatment.

2. Fordyce Spots: These are small, painless, pale bumps that are sebaceous glands without hair follicles. They are also benign and do not require treatment.

3. Fungal or Bacterial Infections: Conditions like candidiasis (a yeast infection) can cause white patches and discomfort. Bacterial infections can also lead to similar symptoms, especially if there is an accompanying discharge.

4. Sexually Transmitted Infections (STIs): Certain STIs, such as herpes or genital warts, can present with white spots or lesions on the glans. If there is any possibility of exposure to STIs, it is crucial to seek medical evaluation.

5. Dermatitis: Contact dermatitis from irritants or allergens can cause redness, itching, and the formation of bumps or spots.

Given your symptoms, it is advisable to avoid using any harsh soaps or body washes on the area until you have a clearer understanding of the cause. Instead, gentle cleansing with warm water is recommended.
For treatment, it is essential to consult a healthcare professional who can perform a physical examination and possibly conduct tests to determine the exact cause of your symptoms. Depending on the diagnosis, treatment may include:
- Topical Antifungals: If a fungal infection is diagnosed, antifungal creams may be prescribed.

- Antibiotics: For bacterial infections, appropriate antibiotics may be necessary.

- Steroid Creams: If inflammation is due to dermatitis, a mild topical steroid may help reduce inflammation and discomfort.

- Proper Hygiene Practices: Maintaining good hygiene and avoiding irritants can help prevent recurrence.

In conclusion, while the symptoms you are experiencing could be due to a variety of causes, it is crucial to seek medical attention for an accurate diagnosis and appropriate treatment. Avoid self-diagnosing or treating with over-the-counter products without professional guidance, as this could potentially worsen the condition. Remember, early intervention is key to effective treatment and relief from symptoms.
