Constant urge to urinate?

Hello, since this afternoon, I've been experiencing a persistent urge to urinate, but when I go to the bathroom, I can only pass a drop or two.
If I exert pressure on my abdomen, I feel a slight soreness.
I want to urinate but can't seem to do so.
When I wipe the genital area with toilet paper, I notice a faint hint of blood, although it's not very obvious.
However, after returning to my office from the restroom, I feel the urge to go again.
This cycle has been repeating throughout the day, and it's quite distressing.
Could you please advise me on any solutions? Thank you.

Liu, 20~29 year old female. Ask Date: 2015/12/14

Dr. Lin Wenbin reply Obstetrics and Gynecology

Quickly see a doctor for examination and treatment; it is likely a bladder infection.

More Info

The symptoms you are experiencing, such as a constant urge to urinate with only a small amount of urine being produced, abdominal discomfort, and the presence of a slight blood tinge when wiping, could indicate a few potential underlying issues. This condition is often associated with urinary tract infections (UTIs), bladder irritation, or other urological conditions.

Possible Causes
1. Urinary Tract Infection (UTI): UTIs are common and can cause symptoms like frequent urination, urgency, and sometimes blood in the urine (hematuria). The infection can irritate the bladder and urethra, leading to discomfort and a feeling of incomplete emptying.

2. Bladder Irritation: Conditions such as interstitial cystitis or overactive bladder can lead to similar symptoms. In these cases, the bladder becomes sensitive, causing frequent urges to urinate with little output.

3. Kidney Stones: If there are stones in the urinary tract, they can cause irritation and lead to symptoms like those you describe, including pain and blood in the urine.

4. Prostatitis: In males, inflammation of the prostate can also lead to urinary symptoms, including urgency and discomfort.

Recommended Actions
1. Hydration: Drink plenty of fluids to help flush out any potential infection. However, avoid irritants such as caffeine, alcohol, and spicy foods, which can exacerbate bladder irritation.

2. Medical Evaluation: It is crucial to consult a healthcare provider as soon as possible. They may perform a urinalysis to check for signs of infection, blood, or other abnormalities. Depending on the findings, further imaging studies or a referral to a urologist may be necessary.

3. Antibiotics: If a UTI is diagnosed, antibiotics will likely be prescribed to clear the infection. It’s important to complete the full course of antibiotics even if symptoms improve.

4. Pain Management: Over-the-counter pain relievers like acetaminophen or ibuprofen can help alleviate discomfort. However, consult with a healthcare provider before taking any medication, especially if you have underlying health conditions.

5. Follow-Up: If symptoms persist despite treatment, further evaluation may be needed to rule out other conditions such as bladder stones or interstitial cystitis.

When to Seek Immediate Care
You should seek immediate medical attention if you experience any of the following:
- Severe abdominal or back pain
- Fever or chills
- Nausea or vomiting
- Increased blood in urine
- Difficulty urinating or complete inability to urinate

Conclusion
Your symptoms are concerning and warrant prompt medical evaluation. While there are several potential causes, a healthcare provider can help determine the exact issue and appropriate treatment. Early intervention is key to preventing complications and ensuring a swift recovery. Please prioritize scheduling an appointment with your healthcare provider to address these symptoms effectively.
