Foreskin issues
Hello doctor, my question is that when I am not erect, the foreskin can fully retract without any discomfort.
However, when I am erect, the foreskin can retract to expose the entire glans, but it can only retract to the coronal sulcus.
Retracting it further causes pain.
Why is this happening?

Dr. Xu Weikai reply Urology
Hello, it seems that the length is not an issue.
It may be caused by insufficient lubrication.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

More Info
Understanding foreskin issues, particularly pain during erection, can be a complex topic that involves anatomical, physiological, and sometimes psychological factors. Based on your description, it seems you are experiencing a condition known as phimosis, which is characterized by the inability to retract the foreskin fully over the glans (the head of the penis) during an erection without experiencing pain.
Why Does This Happen?
1. Anatomical Structure: The foreskin is a fold of skin that covers the glans. In some individuals, the foreskin may be naturally tighter or have less elasticity, making it difficult to retract fully during an erection. This tightness can lead to discomfort or pain when attempting to pull the foreskin back beyond a certain point, such as the coronal sulcus (the groove where the glans meets the shaft of the penis).
2. Scar Tissue or Inflammation: If there has been any history of inflammation, infection, or trauma to the area, scar tissue may form, further restricting the movement of the foreskin. Conditions like balanitis (inflammation of the glans) can also contribute to sensitivity and pain during retraction.
3. Frenulum Issues: The frenulum is a band of tissue on the underside of the penis that connects the foreskin to the glans. If this tissue is short or tight, it can cause pain during erection when the foreskin is pulled back, as it may not allow for adequate movement.
4. Psychological Factors: Sometimes, anxiety or fear related to sexual activity can exacerbate the perception of pain. If you are concerned about the pain, it may lead to increased tension in the pelvic muscles, which can further complicate the issue.
Recommendations for Management
1. Gentle Stretching: If you are comfortable, you might consider gently stretching the foreskin over time to increase its elasticity. This should be done cautiously and without causing pain. It may be beneficial to do this during a warm bath when the skin is more pliable.
2. Topical Treatments: Over-the-counter topical corticosteroids can sometimes help reduce inflammation and increase the elasticity of the foreskin. However, you should consult with a healthcare provider before starting any treatment.
3. Consult a Urologist: Given your symptoms, it would be wise to consult a urologist. They can provide a thorough examination and may recommend treatments such as:
- Circumcision: This is a surgical procedure to remove the foreskin entirely, which can eliminate the issue altogether.
- Preputioplasty: This is a less invasive procedure that involves widening the opening of the foreskin without complete removal.
4. Avoiding Irritants: Ensure that you are not using any harsh soaps or products that could irritate the area. Gentle cleansing with water is usually sufficient.
5. Monitoring Symptoms: Keep track of any changes in your symptoms, including the frequency and intensity of pain. This information can be valuable for your healthcare provider.
Conclusion
Pain during erection due to foreskin issues can be distressing, but it is a condition that can often be managed effectively with the right approach. It is essential to seek professional medical advice to determine the best course of action tailored to your specific situation. Remember, you are not alone in this, and many individuals experience similar issues that can be addressed with proper care and treatment.
