Recent and longstanding issues?
Hello, Doctor: I would like to ask if it is normal for the glans to appear slightly dark after showering, as I have no other symptoms.
Additionally, since middle school, I have needed to stand for a moment before I can urinate, and I find it even more difficult to urinate when others are nearby.
I have had an ultrasound and urinalysis, both of which showed no issues.
Could there be other reasons for this? Regarding masturbation, I would like to know if the duration before finishing falls under the category of premature ejaculation.
Thank you, Doctor.
Li, 10~19 year old female. Ask Date: 2015/12/18
Dr. Xu Wencang reply Urology
The glans appears to be normal.
The difficulty in urination may be due to excessive tension, preventing the sphincter from relaxing.
Premature ejaculation should be assessed based on actual sexual activity.
It is advisable to consult an outpatient clinic for examination and inquiries.

Hello! Thank you for reaching out with your concerns regarding the darkened glans and urination issues. Let's break down your questions and provide some clarity on these matters.
Firstly, regarding the darkening of the glans (the head of the penis), it is important to note that variations in skin color can occur for a variety of reasons. The skin on the glans can appear darker due to increased melanin production, which can be influenced by factors such as friction, irritation, or even hormonal changes. If there are no accompanying symptoms such as pain, itching, or discharge, this may not be a cause for concern. However, if the discoloration persists or is accompanied by other symptoms, it would be wise to consult a urologist for a thorough examination to rule out any underlying conditions such as infections or skin disorders.
Now, regarding your urination issues, it seems that you have experienced difficulty urinating, particularly in social situations or when someone is nearby. This could be related to a psychological condition known as paruresis, or social anxiety related to urination. This condition is characterized by a fear of urinating in public or in the presence of others, which can lead to significant distress and avoidance behaviors. Since you mentioned that you have undergone ultrasound and urinalysis without any findings, it suggests that there may not be a physiological cause for your urination difficulties. Behavioral therapy or cognitive-behavioral therapy (CBT) can be effective in addressing paruresis, helping you to manage anxiety and improve your ability to urinate in various situations.
As for your concerns about masturbation and whether it relates to premature ejaculation, it is important to understand that the frequency and duration of masturbation can vary widely among individuals. Premature ejaculation is typically defined as ejaculation that occurs with minimal sexual stimulation before, during, or shortly after penetration, and it can lead to distress for both partners. If you feel that your sexual experiences are not meeting your expectations or are causing you concern, it may be beneficial to discuss this with a healthcare provider or a sexual health specialist. They can provide guidance and potential treatment options tailored to your needs.
In summary, while the darkening of the glans may not be a significant issue if there are no other symptoms, it is always best to consult a healthcare professional for a definitive diagnosis. Your urination difficulties may be linked to psychological factors rather than physical ones, and addressing these through therapy could be beneficial. Lastly, if you have concerns about your sexual health, seeking advice from a specialist can help clarify any misconceptions and provide you with the support you need.
If you have any further questions or concerns, please feel free to ask. Your health and well-being are important, and seeking clarity on these issues is a positive step forward.
