Bleeding during pregnancy
I am currently 30 weeks pregnant and occasionally feel tightness in my lower abdomen, with my belly feeling hard.
Sometimes, fetal movements are uncomfortable or even painful.
Four days ago, I noticed dark red bleeding, and it has continued despite taking medication.
I am unsure what these symptoms mean.
Could they affect the fetus? How should I handle this situation?

Dr. Wang Hanzhou reply Obstetrics and Gynecology
Occasional tightening in the lower abdomen is likely a natural contraction, but if there are regular contractions accompanied by pain, medical attention should be sought immediately.
At 30 weeks of pregnancy, any bleeding should prompt an evaluation to rule out the possibility of placenta previa.

Bleeding during pregnancy can be a concerning symptom, especially as you approach the third trimester. It's essential to understand the potential causes, implications, and necessary actions to take in such situations.
Firstly, experiencing dark red bleeding at 30 weeks pregnant can indicate several things. It could be a sign of placental issues, such as placental abruption (where the placenta detaches from the uterus) or placenta previa (where the placenta covers the cervix). Both conditions can pose risks to both the mother and the fetus. The tightening of the abdomen and discomfort during fetal movement can also be related to these conditions or could simply be Braxton Hicks contractions, which are normal practice contractions that can occur as the body prepares for labor.
Given that you have been experiencing continuous bleeding after taking medication, it is crucial to seek immediate medical attention. Continuous bleeding can lead to complications, including preterm labor or fetal distress, and it is essential to determine the source of the bleeding through ultrasound and other diagnostic tests. Your healthcare provider may also want to monitor the fetal heart rate and your overall condition to ensure that both you and your baby are safe.
In terms of how this bleeding might affect your baby, it largely depends on the cause and severity of the bleeding. If the bleeding is due to a placental issue, it could potentially lead to complications such as low birth weight, preterm birth, or in severe cases, stillbirth. However, many women with bleeding during pregnancy go on to have healthy pregnancies and deliveries, especially with appropriate medical care.
As for managing your symptoms, it is essential to follow your doctor's advice closely. If you are advised to rest, take that seriously, as physical activity can exacerbate bleeding. Avoid heavy lifting, strenuous exercise, and sexual intercourse until you have been cleared by your healthcare provider. Staying hydrated and maintaining a balanced diet can also support your overall health during this time.
Additionally, keep a close watch on your symptoms. If you experience increased bleeding, severe abdominal pain, or any signs of labor (such as contractions), it is vital to seek emergency medical care.
In summary, while some bleeding can be normal in pregnancy, especially in the early stages, bleeding at 30 weeks requires immediate evaluation. The symptoms you describe warrant a thorough examination by your healthcare provider to rule out any serious conditions and to ensure the safety of both you and your baby. Always prioritize communication with your healthcare team and do not hesitate to reach out with any concerns.
