Scars on the neck?

Hello, Doctor: I have a history of urticaria since childhood.
Last week, I developed a large patch on my neck while sleeping, and I may have scratched it during the night, which caused it to turn into many small bumps on half of my neck.
They haven't completely disappeared yet.
I picked up some ointment for urticaria last week.
I would like to know how long it will take for these small bumps to go away and whether they will leave scars.
Thank you!

Yumi, 20~29 year old female. Ask Date: 2015/12/21

Dr. Li Yufen reply Dermatology

Hello: The duration for the rash to resolve can vary from person to person.
It is recommended to continue applying the medication and to avoid scratching to prevent the formation of wounds that may leave scars.
Thank you!

When dealing with hives (urticaria), it is important to understand both the healing process and the potential for scarring. Hives are typically characterized by raised, itchy welts on the skin that can vary in size and shape. They are often a result of an allergic reaction, stress, or other triggers. In your case, it seems that scratching the hives may have exacerbated the condition, leading to further irritation and possibly the formation of small bumps.

Healing Time for Hives
The healing time for hives can vary significantly depending on several factors, including the severity of the reaction, the underlying cause, and individual skin healing characteristics. Generally, hives can resolve within a few hours to a few days if the trigger is removed and appropriate treatment is administered. However, in some cases, hives can persist for weeks or even longer, especially if they are chronic or recurrent.

In your situation, since you have a history of hives and have recently experienced a flare-up, it is essential to continue using the prescribed medication, such as topical corticosteroids or antihistamines, to manage the symptoms and reduce inflammation. If the hives do not improve within a week or if they worsen, it is advisable to consult your healthcare provider for further evaluation and potential adjustments to your treatment plan.

Potential for Scarring
As for the potential for scarring, it is generally uncommon for hives themselves to leave permanent scars. However, if the skin is scratched excessively, as you mentioned, this can lead to secondary skin changes, such as post-inflammatory hyperpigmentation or even scarring in some cases. The risk of scarring increases with deeper skin damage, such as from vigorous scratching or if the skin becomes infected.

To minimize the risk of scarring, consider the following tips:
1. Avoid Scratching: Try to resist the urge to scratch the affected areas. Scratching can lead to further irritation and potential skin damage.

2. Keep the Area Moisturized: Use a gentle, fragrance-free moisturizer to keep the skin hydrated. This can help soothe the skin and promote healing.

3. Use Cold Compresses: Applying a cold compress to the affected area can help reduce itching and inflammation, making it less likely for you to scratch.

4. Follow Treatment Recommendations: Continue using any prescribed medications as directed. If you notice any signs of infection (such as increased redness, warmth, or pus), seek medical attention promptly.

5. Sun Protection: Protect the healing skin from sun exposure, as UV rays can exacerbate pigmentation changes and increase the risk of scarring.

6. Consult a Dermatologist: If you are concerned about the appearance of your skin or if the hives persist, consider consulting a dermatologist. They can provide specialized treatment options and advice tailored to your situation.

Conclusion
In summary, while hives can be uncomfortable and distressing, they typically resolve without leaving scars, especially if proper care is taken. The healing time can vary, but with appropriate treatment and self-care measures, you should see improvement. If you have ongoing concerns or if the condition worsens, do not hesitate to reach out to your healthcare provider for further guidance. Remember, managing the underlying triggers and taking care of your skin are key to preventing future flare-ups and complications.
