Can pregnant women use streptomycin?

I stubbed my thumb on the table, and as a result, my nail was lifted.
I went to see a doctor, who prescribed me a topical antibiotic ointment.
However, I'm unsure if it's safe for pregnant women to use.
Although the doctor said it is harmless to the baby, I’m still concerned.
Are there any side effects?

Dr. Liao Huiling reply Pharmacology

Topical medications used locally will not have adverse effects on the fetus, so please use them with confidence.

When it comes to the use of Gentamicin ointment during pregnancy, it is essential to consider both the potential benefits and risks associated with its application. Gentamicin is an aminoglycoside antibiotic that is commonly used to treat various bacterial infections. However, its safety profile during pregnancy is a topic of concern for many expectant mothers.

According to the FDA (Food and Drug Administration) pregnancy categories, Gentamicin is classified as a Category D drug. This classification indicates that there is evidence of human fetal risk based on adverse reaction data, but the potential benefits of the drug may warrant its use in pregnant women despite these risks. In practical terms, this means that while Gentamicin can be effective in treating infections, it should only be used when absolutely necessary and under the guidance of a healthcare provider.

One of the primary concerns with Gentamicin, especially when used systemically (such as through injections), is its potential for ototoxicity (hearing damage) and nephrotoxicity (kidney damage). These risks are particularly heightened with prolonged use or high doses. However, when Gentamicin is applied topically, as in the case of ointment, the systemic absorption is significantly lower, which may reduce the risk of these adverse effects. Nevertheless, some absorption can still occur, and caution is advised.

In your case, since you mentioned that the doctor assured you that the ointment is safe for your baby, it is crucial to follow their guidance. The doctor likely weighed the risks and benefits before prescribing the medication. If the ointment is being used to treat a localized infection, the risk to the fetus is generally considered lower compared to systemic administration. However, it is always wise to monitor for any unusual symptoms or side effects.

If you have concerns about using Gentamicin ointment, consider discussing them further with your healthcare provider. They can provide you with more personalized advice based on your medical history and the specifics of your pregnancy. Additionally, if you experience any adverse reactions or if your condition does not improve, it is essential to seek medical attention promptly.

In summary, while Gentamicin ointment is classified as a Category D drug, its topical use may pose a lower risk to the fetus compared to systemic use. Always consult with your healthcare provider before using any medication during pregnancy, and ensure that you are closely monitored for any potential side effects. Your health and the health of your baby are of utmost importance, and your healthcare provider is the best resource for making informed decisions regarding medication use during pregnancy.
