Postoperative care after anal fistula surgery?

Hello Dr.
Ke, I had anal fistula surgery 37 days ago, and I still have yellow-green discharge with some slight bleeding.
A few days ago, my stool was harder, and I was wondering if that could affect the wound? The wound hasn't completely healed yet, and during my follow-up appointment, I asked my attending physician why the healing is taking so long.
They mentioned that my fistula opening is deeper, is this normal?

Dr. Ke Fangxu reply Surgery

It is common for deeper complex fistula surgeries to have a longer healing time.
It is recommended to avoid constipation until the wound is completely healed.
Taking warm sitz baths can also aid in wound recovery.
If necessary, stool softeners can be taken to prevent hard stools from causing secondary tearing of the wound.

After anal fistula surgery, it is common to experience some discharge and healing challenges, especially in the initial weeks following the procedure. In your case, at 37 days post-surgery, the presence of yellow-green discharge with slight bleeding can be concerning, but it is not uncommon. The healing process for anal fistulas can vary significantly from person to person, and several factors can influence the duration and quality of recovery.

Firstly, the nature of the fistula itself plays a crucial role in healing. If the fistula was particularly deep or complex, as your doctor indicated, it may take longer to heal completely. The depth and complexity of the fistula can affect how well the tissue can regenerate and close up. Additionally, the surgical technique used can also impact recovery time. Some techniques may leave larger wounds that take longer to heal.

Regarding the discharge you are experiencing, it is essential to monitor its characteristics. Yellow-green discharge can indicate the presence of infection, especially if it is accompanied by pain, increased swelling, or a fever. However, some discharge is normal as the body works to heal the surgical site. The slight bleeding you mentioned, particularly after having hard stools, can be attributed to trauma to the healing tissue. Hard stools can cause strain during bowel movements, leading to irritation or injury to the surgical site. It is crucial to maintain soft stools to minimize this risk. A high-fiber diet, adequate hydration, and possibly stool softeners can help in this regard.

In terms of healing time, while many patients may start to see significant improvement within a few weeks, complete healing of an anal fistula can take several weeks to months, depending on individual circumstances. If your doctor has indicated that your healing is slower than expected, it is essential to follow up with them regularly. They may recommend additional treatments, such as sitz baths, topical medications, or even further interventions if necessary.

If you are concerned about the healing process or if the discharge worsens, it is advisable to reach out to your healthcare provider. They may want to assess the wound to ensure there are no complications, such as an abscess or persistent infection. Regular follow-ups are crucial in managing your recovery effectively.

In summary, while some discharge and slow healing can be normal after anal fistula surgery, it is essential to monitor your symptoms closely. Maintaining soft stools and following your doctor's advice on wound care can significantly aid in your recovery. If you have any concerns or if your symptoms change, do not hesitate to contact your healthcare provider for further evaluation and guidance.
