Post-Operative Discharge: What to Expect After a D&C - Obstetrics and Gynecology

Postoperative discharge?

Hello Dr.
Lin, I would like to ask about my recent abortion procedure on December 11.
During my follow-up appointment last week, the doctor mentioned that the lochia was nearly finished, but I am still experiencing a significant amount of thick, dark brown discharge.
How long can I expect the discharge to last after the lochia has stopped? Is it normal for the discharge to have a slight pink tint?

fayun, 20~29 year old female. Ask Date: 2015/12/25

Dr. Lin Wenbin reply Obstetrics and Gynecology

A surgical specialist needs to conduct an examination.

After a dilation and curettage (D&C) procedure, it is common for patients to experience vaginal discharge, known as lochia. This discharge can vary in color, consistency, and duration, depending on individual healing processes and the specifics of the procedure performed.

Typically, lochia can be categorized into three stages:
1. Lochia Rubra: This is the initial discharge, which is bright red and consists mainly of blood. It usually lasts for about 3 to 4 days post-procedure.

2. Lochia Serosa: Following the rubra stage, the discharge transitions to a pinkish-brown color, indicating that the bleeding is subsiding. This stage can last from about 4 to 10 days.

3. Lochia Alba: This is the final stage, characterized by a yellowish-white discharge, which can last for several weeks, typically up to 4 to 6 weeks after the procedure.

In your case, since you are experiencing a thick, coffee-colored discharge, it is likely that you are in the lochia serosa stage. The presence of some pinkish hue in the discharge can be normal as the body continues to heal. However, it is essential to monitor the discharge for any changes in color, odor, or consistency. If the discharge becomes foul-smelling, or if you experience heavy bleeding (soaking through a pad in an hour), fever, or severe abdominal pain, you should contact your healthcare provider immediately, as these could be signs of infection or other complications.

The duration of lochia can vary significantly among individuals. Factors such as the type of procedure performed, your overall health, and whether there were any complications can influence how long you experience discharge. Generally, it is not unusual for some women to have discharge for several weeks after a D&C.

In summary, a pinkish discharge post-D&C can be a normal part of the healing process. However, if you have concerns about the amount or appearance of the discharge, or if you experience any unusual symptoms, it is advisable to follow up with your healthcare provider for further evaluation and reassurance. They can provide personalized advice based on your specific situation and medical history.
