Testicular displacement sensation
Hello doctor, I was previously diagnosed with varicocele.
Recently, after sexual intercourse, when I lie down to sleep, I experience a sensation of my left testicle retracting upwards after a few minutes.
I have tried applying cold water, but I'm not sure if it has any direct effect; the sensation usually subsides after a while.
When I palpate the testicle, it is still in its normal position.
What could this symptom indicate? What potential effects could it have?

Dr. Xu Weikai reply Urology
Steven: Hello, I can only speculate that it is likely due to the contraction of the cremaster muscle, which causes the testes to move upward.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

The sensation of testicular displacement or movement after sexual activity can be concerning, especially if you have a history of conditions like varicocele (enlargement of veins within the scrotum). The feeling that your left testicle is "running up" or moving can be attributed to several factors, including anatomical, physiological, and psychological aspects.
Firstly, it’s important to understand the anatomy of the male reproductive system. The testicles are housed in the scrotum, which is designed to keep them at a temperature slightly lower than the body’s core temperature for optimal sperm production. The cremaster muscle, which surrounds the testicles, can contract and relax in response to various stimuli, including temperature changes, sexual arousal, and physical activity. This contraction can cause the testicles to move closer to the body, which may explain the sensation you are experiencing after sexual activity.
The sensation of your testicle moving upwards could also be linked to the physiological responses during sexual arousal and orgasm. During sexual excitement, the body undergoes various changes, including increased blood flow and muscle contractions, which can affect the position of the testicles. After ejaculation, the body begins to relax, and the muscles may also relax, leading to a sensation of movement or displacement.
Another factor to consider is the psychological aspect. Anxiety or concern about your testicular health, especially with a previous diagnosis of varicocele, can heighten your awareness of sensations in that area. This heightened awareness can lead to a perception of movement or discomfort that may not be as pronounced if you were feeling more relaxed.
Regarding the impact of this sensation, if the testicles are not actually moving out of their normal anatomical position and there is no accompanying pain, swelling, or other concerning symptoms, it is likely not a cause for alarm. However, if you experience persistent discomfort, pain, or any changes in the appearance of the testicles, it would be prudent to consult a healthcare professional for a thorough evaluation.
In terms of management, applying cold compresses, as you mentioned with ice water, can help alleviate any discomfort or swelling that may arise after sexual activity. This is a common practice to reduce inflammation and provide relief. Additionally, wearing supportive underwear can help minimize movement and provide comfort.
If this sensation continues or worsens, or if you develop any new symptoms such as pain, swelling, or changes in urination, it would be wise to seek medical attention. A healthcare provider can perform a physical examination and may recommend imaging studies, such as an ultrasound, to rule out any underlying issues such as a hernia or complications related to varicocele.
In conclusion, while the sensation of testicular displacement after sexual activity can be disconcerting, it is often a benign phenomenon related to normal physiological responses. However, staying vigilant about any changes in your body and seeking medical advice when in doubt is always a good practice for maintaining reproductive health.
