Menstrual irregularities
Hello, my menstrual cycle has always been regular, but from November 14th until now, January 2nd, I still haven't gotten my period.
I've been experiencing abdominal pain similar to when my period is about to start, and my breasts feel a bit swollen.
However, I bought a pregnancy test and it came back negative (my husband and I have not been using any contraception).
So, doctor, could there be a problem with me?
Miss Zhan, 40~49 year old female. Ask Date: 2016/01/02
Dr. Lin Manying reply Obstetrics and Gynecology
If the menstrual cycle is longer than 35 days and a pregnancy test is negative, it is recommended to visit a gynecologist for an examination to check for any issues or to determine if it is a temporary phenomenon.
Ongoing treatment with medication may be necessary.

More Info
Menstrual irregularities can be concerning, especially when you have previously experienced regular cycles. The situation you described, where your period has not arrived since November 14 and you are experiencing symptoms such as abdominal pain and breast tenderness, can be attributed to several potential causes.
Firstly, it is important to consider the possibility of pregnancy, especially since you mentioned that you and your husband have not been using contraception. While you have taken a home pregnancy test that returned negative, it is worth noting that these tests can sometimes yield false negatives, particularly if taken too early. If your period does not arrive soon, it may be advisable to repeat the test after a week or consult with your healthcare provider for a blood test, which is more sensitive and can detect pregnancy earlier.
If pregnancy is ruled out, there are several other factors that could contribute to your menstrual irregularities. Stress is a significant factor that can disrupt hormonal balance and lead to missed periods. Emotional or physical stressors, including changes in your lifestyle, work, or personal life, can impact your menstrual cycle.
Another common cause of missed periods is hormonal imbalances, which can be due to conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or other endocrine issues. PCOS, in particular, is characterized by irregular menstrual cycles, weight gain, and sometimes symptoms like acne or excessive hair growth. Thyroid dysfunction, whether hypothyroidism or hyperthyroidism, can also lead to changes in menstrual patterns.
Weight fluctuations can also play a role in menstrual irregularities. Significant weight loss or gain can affect your body's hormonal balance, leading to missed periods. If you have recently changed your diet or exercise routine, this could be a contributing factor.
Additionally, certain medications or contraceptives can impact your menstrual cycle. If you have started or stopped any medications recently, this could be a reason for the changes you are experiencing.
Given your symptoms and the duration of your missed period, it is advisable to consult with a healthcare provider. They can perform a thorough evaluation, including a physical examination, blood tests to check hormone levels, and possibly imaging studies if necessary. This will help to identify any underlying conditions that may be contributing to your menstrual irregularities.
In summary, while there are several potential causes for your missed period, including pregnancy, stress, hormonal imbalances, weight changes, and medication effects, it is essential to seek medical advice for a proper diagnosis and management plan. Your healthcare provider will be able to guide you through the necessary steps to address your concerns and restore your menstrual health.
