Chronic Epididymitis: Symptoms, Treatment, and Fertility Concerns - Urology

Epididymitis has been an issue for a long time?

Hello, I developed symptoms in September 2014.
After receiving injections and taking antibiotics, I recently had an ultrasound last month, which indicated that I have epididymitis on the left side.
Despite receiving intravenous treatment and medication, my condition has not improved; the pain is just slightly less than before.
Am I experiencing chronic epididymitis? How should I manage this? Is it treatable? I have undergone all necessary tests, and there were no bacteria or underlying issues found.
My semen analysis is normal, and my blood tests are fine.
However, the ultrasound results indicate epididymitis, and I experience pain about five times a day on both sides.
I have been on medication and received injections for over a year now.
A few days ago, I felt relatively fine, but today the pain has returned, affecting both sides.
I am at a loss as to what is happening.
I have been on medication to the point where even my doctor advised stopping, and it has been very challenging.
I am also concerned about whether this will affect my fertility.
Additionally, I have some pain on the glans.
What should I do?

Xiao Liang, 20~29 year old female. Ask Date: 2016/01/04

Dr. Xu Weikai reply Urology

Xiao Liang: Hello, a diagnosis of chronic epididymitis is made when symptoms persist for more than six months without signs of bacterial infection.
If epididymitis becomes chronic, it may result in permanent enlargement, but severe pain is uncommon; instead, there may be mild discomfort or swelling.
If there is ongoing severe pain, it could indicate the presence of bacteria, suggesting an acute bacterial infection is still active, or it could be a recurrence of acute infection during the chronic phase.
Based on your situation, my preliminary assessment is that you may have another infection.
To determine if this affects fertility, a semen analysis can be performed.
If there are any unclear points, feel free to ask again for further discussion.

Chronic epididymitis is a condition characterized by inflammation of the epididymis, which is the tube that carries sperm from the testicles. This condition can be quite distressing, especially when it persists for an extended period, as in your case since 2014. Understanding the symptoms, treatment options, and potential fertility concerns associated with chronic epididymitis is crucial for managing your health effectively.

Symptoms of Chronic Epididymitis
The symptoms of chronic epididymitis can vary but often include:
1. Persistent Pain: This is usually a dull ache in the scrotum or testicles that can fluctuate in intensity. You mentioned experiencing pain on both sides, which is common in chronic cases.

2. Swelling: The affected epididymis may become swollen and tender.

3. Discomfort During Sexual Activity: Pain may increase during ejaculation or sexual intercourse.

4. Changes in Urination: Some individuals may experience increased urgency or frequency of urination.

5. Glandular Symptoms: Occasionally, there may be associated symptoms like fever or malaise, although these are less common in chronic cases.

Diagnosis and Treatment
Given that you have undergone various tests, including ultrasound and blood tests, and have not found any bacterial infection, it is likely that your condition is classified as chronic non-bacterial epididymitis. This can occur due to several factors, including:
- Previous Infections: Even if the initial infection has resolved, inflammation can persist.

- Trauma or Injury: Physical trauma to the groin area can lead to chronic pain.

- Autoimmune Responses: In some cases, the body may mistakenly attack its own tissues.

- Pelvic Floor Dysfunction: Issues with the pelvic floor muscles can contribute to pain.

Treatment Options
1. Pain Management: Non-steroidal anti-inflammatory drugs (NSAIDs) can help alleviate pain and reduce inflammation. Your doctor may recommend medications like ibuprofen or naproxen.

2. Physical Therapy: Pelvic floor physical therapy can be beneficial for managing chronic pain and improving muscle function.

3. Warm Compresses: Applying warm compresses to the affected area can provide relief.

4. Avoiding Irritants: Reducing activities that may exacerbate pain, such as heavy lifting or prolonged sitting, can be helpful.

5. Surgery: In rare cases, if conservative treatments fail, surgical options may be considered to remove the epididymis (epididymectomy) or to address any anatomical issues.

Fertility Concerns
Regarding fertility, chronic epididymitis can potentially affect sperm transport, but many men with this condition maintain normal fertility, especially if sperm production is not impacted. Since your semen analysis has returned normal results, it suggests that your sperm production is likely unaffected. However, if you have concerns about fertility, it may be beneficial to consult a urologist who specializes in male reproductive health for further evaluation.

Conclusion
Chronic epididymitis can be a challenging condition to manage, especially when symptoms persist despite treatment. It is essential to maintain open communication with your healthcare provider about your symptoms and treatment options. If your current treatment plan is not providing relief, consider seeking a second opinion or consulting a specialist in urology or chronic pain management. They may offer additional insights or alternative therapies that could improve your quality of life. Remember, while chronic conditions can be frustrating, many patients find effective ways to manage their symptoms and lead fulfilling lives.
