Shoulder and back pain
After the car accident, my arm starts to hurt by noon, and my left shoulder and back also experience pain, with the worst symptoms occurring around noon.
I have seen both orthopedic and neurology specialists; some are unsure of what to do, while others suggest that it may be brachial plexus nerve damage.
Only one rehabilitation physician privately used an ultrasound and indicated that there is a muscle tear...
(however, I don't understand why this finding did not appear in the official report).
Could brachial plexus nerve damage potentially cause these symptoms?
Niming, 20~29 year old female. Ask Date: 2016/01/05
Dr. Lan Shengxing reply Neurology
Brachial plexus injury itself is less likely to cause such pain, but the weakness resulting from the injury may indeed lead to this pain.
Whether the pain originates from muscle or nerve sources may require a detailed medical history to determine.

More Info
After a car accident, experiencing arm pain, especially if it is localized to the shoulder and elbow, can be concerning and may indicate various underlying issues. In your case, it seems that you have already sought medical attention from orthopedic and neurological specialists, which is a good step. However, the inconsistency in the diagnoses you've received can understandably lead to confusion and anxiety about your condition.
First, let's discuss the possibility of nerve damage. The brachial plexus, a network of nerves that sends signals from the spine to the shoulder, arm, and hand, can be affected during traumatic events like car accidents. If the brachial plexus is injured, it can lead to symptoms such as pain, weakness, or numbness in the arm. However, nerve damage typically presents with additional symptoms such as tingling, numbness, or a "pins and needles" sensation, which you haven't mentioned. If you are only experiencing pain without these additional symptoms, it may suggest that the issue is more related to soft tissue injury rather than direct nerve damage.
The pain you are experiencing in your shoulder and elbow could also be attributed to soft tissue injuries, such as a rotator cuff injury or tendonitis, which are common after trauma. The rotator cuff is a group of muscles and tendons that stabilize the shoulder joint, and injuries to this area can lead to pain and limited range of motion. The fact that you are experiencing pain when trying to lift or use your arm suggests that there may be some inflammation or injury to these structures.
In your case, the ultrasound performed by the rehabilitation physician indicating a muscle tear could explain the persistent pain and weakness. Muscle tears can occur without visible fractures on X-rays and may not always be documented in standard reports unless specifically assessed. If a muscle tear is present, it can lead to significant discomfort and functional limitations, especially when attempting to lift or carry objects.
Given that you have already consulted with specialists, the next steps should include further imaging studies, such as an MRI, to get a clearer picture of the soft tissue structures in your shoulder and elbow. An MRI can help identify any tears in the rotator cuff, tendonitis, or other soft tissue injuries that may not be visible on X-rays. Additionally, it may be beneficial to continue with physical therapy, as rehabilitation can help strengthen the muscles around the shoulder and elbow, improve range of motion, and reduce pain.
If you are experiencing severe pain or if your symptoms worsen, it may be necessary to revisit the emergency room or consult with a specialist who can provide a more comprehensive evaluation. It is essential to address these symptoms promptly to prevent any long-term complications or chronic pain issues.
In summary, while nerve damage is a possibility, the symptoms you describe may be more indicative of soft tissue injuries such as a rotator cuff tear or tendonitis. Further imaging studies and a thorough evaluation by a specialist are crucial in determining the exact cause of your pain and developing an appropriate treatment plan. Remember to communicate openly with your healthcare providers about your symptoms and concerns to ensure you receive the best possible care.
