Straining during bowel movements?

Hello doctor, sometimes I feel the urge to have a bowel movement but can't seem to pass it.
At times, it feels like it's almost ready to come out but doesn't, so I have to strain.
When I strain, sometimes I also urinate involuntarily.
When I don't strain, sometimes I end up urinating along with the bowel movement.
Is this normal? I don't experience any leakage when I laugh or exert myself, only during bowel movements.
I read online that walking on tiptoes can help train the bladder muscles; is that true?

ray, 20~29 year old female. Ask Date: 2016/01/06

Dr. Xu Weikai reply Urology

Ray: Hello, young person.
Generally, bladder issues are rare at your age.
Your description of walking on your toes seems normal, and I don't think it poses any harm.
As for whether it benefits the bladder, I'm not sure.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

Straining during bowel movements is a common issue that many people experience, and it can lead to various symptoms, including the involuntary release of urine. This phenomenon can be concerning, but it is essential to understand the underlying mechanisms and whether it is considered normal.

When you strain to have a bowel movement, you increase intra-abdominal pressure. This pressure can affect the pelvic floor muscles, which support both the bladder and the rectum. If these muscles are weak or if there is a lack of coordination between them, straining can lead to the involuntary release of urine. This is particularly common in individuals who may have pelvic floor dysfunction or weakness, which can be exacerbated by factors such as age, childbirth, or certain medical conditions.

In your case, experiencing urine leakage only during bowel movements and not during other activities like laughing or exerting yourself suggests that the issue may be related to the specific mechanics of straining and the pelvic floor's response during that time. It is not uncommon for people to experience this, especially if they have to exert significant effort to pass stool.

Regarding the idea of using a footstool or "squatty potty" to elevate your feet while sitting on the toilet, this practice can indeed help facilitate bowel movements. Elevating the feet can change the angle of the rectum, making it easier to pass stool. This position can also help relax the pelvic floor muscles, potentially reducing the need to strain. However, while this may improve bowel movements, it may not directly address the issue of urinary leakage during straining.

To address your concerns more effectively, consider the following recommendations:
1. Pelvic Floor Exercises: Engaging in pelvic floor exercises, commonly known as Kegel exercises, can strengthen the muscles that support the bladder and rectum. These exercises involve contracting and relaxing the pelvic floor muscles and can help improve coordination and strength, potentially reducing involuntary leakage during bowel movements.

2. Dietary Adjustments: Ensure that your diet is high in fiber to promote regular bowel movements and reduce the need for straining. Staying hydrated is also crucial, as it helps soften stool and makes it easier to pass.

3. Consult a Healthcare Professional: If this issue persists or worsens, it is advisable to consult a healthcare provider, such as a gastroenterologist or a pelvic floor specialist. They can assess your situation more thoroughly and provide tailored advice or treatment options.

4. Avoid Straining: If possible, try to avoid straining during bowel movements. If you find yourself needing to strain frequently, it may be a sign of constipation or another underlying issue that should be addressed.

In summary, while it is not uncommon to experience urinary leakage during straining, it is essential to address the underlying causes. Strengthening the pelvic floor, making dietary changes, and consulting with a healthcare professional can help manage and potentially resolve the issue.
