Recurrent balanitis

Hello, Doctor.
I have a question about how to handle a situation.
At the end of October last year, I had candidal balanitis and saw a dermatologist.
It healed after about a month, but in the past two weeks, I have noticed the following phenomena on my glans: 1.
Occasionally, I find one to two red spots on the glans that are neither painful nor itchy.
After noticing them, I immediately apply the ointment prescribed by the dermatologist, and within about two days, the red spots fade away.
This situation has occurred approximately once or twice a week for the past one to two weeks, repeating two to three times.
I wash the area daily in the morning and during showers, following the doctor's advice to use warm/cool water and avoid soap, cleansers, or body wash.
However, this situation still occurs.
I would like to ask the doctor: 1.
Does this situation indicate a recurrence of balanitis, or could it be due to vascular constriction/dilation causing the skin to show red spots? 2.
What could be causing this situation? 3.
How should I handle this situation? 4.
Is there anything I should pay special attention to?

Hello,
1.
It may be a recurrent issue similar to a cold; you wouldn't say a cold has relapsed.

2.
Usually, it is due to excessive cleaning, which can damage the epidermis.
If possible, apply some petroleum jelly after cleaning.

3.
Strengthening the care of the glans is important.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
Answers found online should only be used as a reference; if necessary, it is still important to seek medical attention at a healthcare facility.

Best regards,
Dr.

Managing recurrent balanitis, particularly when it involves candidal infections (commonly known as yeast infections), can be challenging. Based on your description, it seems that you have experienced recurrent symptoms after an initial episode of balanitis, which was treated successfully. Here’s a comprehensive overview of the potential causes, symptoms, and treatment options for managing recurrent balanitis.

Understanding Balanitis
Balanitis is an inflammation of the glans (the head) of the penis, which can be caused by various factors, including infections (like yeast infections), irritants, or underlying medical conditions. In your case, the recurrent red spots on the glans that resolve with topical treatment suggest that you may be experiencing a mild form of balanitis, possibly due to a candidal infection.

Causes of Recurrent Balanitis
1. Infections: The most common cause of balanitis is a fungal infection, particularly from Candida species. Other infectious agents can include bacteria and sexually transmitted infections (STIs).

2. Irritation: Irritants such as soaps, detergents, or even certain fabrics can lead to inflammation. You mentioned that you avoid soaps and use only water, which is good practice.

3. Underlying Conditions: Conditions like diabetes can predispose individuals to recurrent infections due to higher glucose levels in bodily fluids, which can promote yeast growth.

4. Hygiene Practices: While maintaining good hygiene is essential, over-washing or using harsh products can irritate the sensitive skin of the glans.

5. Allergic Reactions: Allergies to latex (from condoms) or other materials can also cause inflammation.

Symptoms
The symptoms of balanitis can include:
- Redness and swelling of the glans
- Itching or irritation (though you mentioned your symptoms are not itchy)
- Discharge or a foul odor (not mentioned in your case)
- Pain during urination or sexual activity (not mentioned in your case)

Treatment Options
1. Topical Antifungals: Since you have had success with a topical medication prescribed by your dermatologist, it may be beneficial to continue using it as directed during flare-ups. Common antifungal creams include clotrimazole or miconazole.

2. Avoid Irritants: Continue to avoid soaps and other potential irritants. Use only water for cleaning and consider using a gentle, hypoallergenic moisturizer if dryness is an issue.

3. Maintain Dryness: Keeping the area dry is crucial, as moisture can promote fungal growth. After bathing, ensure the area is thoroughly dried.

4. Consult a Specialist: If symptoms persist or worsen, it may be advisable to consult a urologist or a dermatologist specializing in genital conditions. They may perform further tests to rule out other underlying conditions or infections.

5. Consider Systemic Factors: If you have underlying conditions such as diabetes, managing those effectively can help reduce the frequency of infections.

Special Considerations
- Monitor Symptoms: Keep a diary of your symptoms, noting when they occur and any potential triggers. This information can be valuable for your healthcare provider.

- Avoid Self-Diagnosis: While it’s tempting to self-treat based on past experiences, it’s essential to have a healthcare professional evaluate any new or persistent symptoms.

- Sexual Health: If you are sexually active, consider discussing your symptoms with your partner and practicing safe sex to prevent potential transmission of infections.

In conclusion, recurrent balanitis can be managed effectively with proper hygiene, avoidance of irritants, and appropriate medical treatment. If your symptoms continue to recur despite these measures, seeking further evaluation from a healthcare professional is crucial to ensure that there are no underlying issues contributing to your condition.
