Falling and landing on the buttocks can lead to unusual erections afterward?

Hello, Dr.
Hsu.
A few days ago, I fell and landed on my buttocks, which caused pain in my lumbar spine for one night.
After several days, I noticed that I no longer have morning erections, and the hardness of my erections has decreased.
I cannot achieve an erection just by thinking about it or watching videos, but I can still achieve one manually, although the hardness is significantly less than before.
Previously, I could achieve a normal erection through fantasy or watching videos, but now I cannot after the fall.
Before, I could achieve an erection before going to sleep, but now I cannot, and even when I do manage to get an erection, there is no sensation.
I no longer have the sensitivity or pleasurable feelings I used to have, and I also do not feel the urge to urinate in the morning.
I went to an orthopedic doctor who scolded me, then to a neurologist who said I need to get an MRI of my spine, and then to a urologist who said they cannot diagnose me.
I am very grateful for the help from all the doctors and appreciate your response, even though the problem has not yet been resolved.

Post-fall erectile issues can be a complex topic, especially when they are accompanied by back pain. The connection between these two symptoms may not be immediately apparent, but there are several physiological and psychological factors at play.

Firstly, it is important to understand that erectile function is influenced by a combination of physical, hormonal, and psychological factors. The fact that you experienced a fall and subsequent back pain could potentially lead to a disruption in these factors. The lumbar spine is closely associated with the pelvic region, and any injury or pain in this area can affect the nerves that are responsible for erectile function. Specifically, the nerves that control erections originate from the lower spinal cord (S2-S4), and any trauma or inflammation in the lumbar region could potentially impact these nerves.

In your case, the acute back pain following the fall may have led to a temporary disruption in nerve function or blood flow to the pelvic area, which could explain the changes in your erectile function. Additionally, pain can lead to increased stress and anxiety, which are known to negatively affect sexual performance. The psychological aspect of erectile dysfunction (ED) is significant; anxiety about performance can create a cycle where the fear of not being able to achieve an erection leads to further difficulties.

Moreover, the absence of morning erections (morning wood) is often a sign that there may be an underlying physiological issue. Morning erections are typically a result of hormonal fluctuations during sleep and are a good indicator of healthy erectile function. The fact that you are experiencing a lack of these could suggest that the issue is not purely psychological but may have a physical component as well.

It is also worth noting that certain medications or treatments for pain management can have side effects that impact sexual function. If you have been prescribed any pain relief medications, it would be prudent to discuss these with your healthcare provider to determine if they could be contributing to your symptoms.

Given the complexity of your situation, it is essential to take a multi-faceted approach to diagnosis and treatment. Here are some recommendations:
1. Follow-Up with Specialists: Since you have already seen orthopedic, neurological, and urological specialists, it may be beneficial to continue following up with them. An MRI of the lumbar spine can help identify any structural issues that may be compressing nerves.

2. Physical Therapy: Engaging in physical therapy may help alleviate back pain and improve mobility, which could, in turn, help restore erectile function. A physical therapist can provide exercises that strengthen the pelvic floor and lower back, potentially improving nerve function and blood flow.

3. Psychological Support: Consider speaking with a mental health professional who specializes in sexual health. They can help address any anxiety or stress related to your erectile issues and provide coping strategies.

4. Lifestyle Modifications: Maintaining a healthy lifestyle, including regular exercise, a balanced diet, and adequate sleep, can improve overall health and potentially enhance erectile function.

5. Medication Review: If you are taking any medications for pain or other conditions, discuss these with your healthcare provider to assess their impact on your erectile function.

In conclusion, the connection between your fall, back pain, and erectile issues is likely multifactorial. It is essential to approach this problem holistically, considering both the physical and psychological aspects. Continued communication with your healthcare providers will be key in finding an effective solution.
