Sometimes there is intermittent pain in the right testicle?
Hello Dr.
Hsu: Recently, I have experienced intermittent pain in my right testicle.
Sometimes it hurts, and sometimes it doesn't.
Today while showering, I felt one or two lumps on the inner side of the right scrotum, but I didn't feel any pain while sleeping.
The testicles are not of different sizes.
Recently, due to my studies, I often go to bed around 1 or 2 AM.
Could this be related?

Dr. Xu Weikai reply Urology
Xiao Ming: Hello, tumors located in the scrotum are not related to staying up late.
If they occur frequently, it is advisable to seek medical attention.
Many hospitals now offer evening outpatient services, which will not interfere with school.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

Intermittent pain in the testicle, particularly when localized to one side, can be concerning and warrants careful consideration of various potential causes. It’s important to understand that the testicles are sensitive organs and can be affected by a range of issues, from benign to more serious conditions.
Firstly, the pain you are experiencing could be related to a condition known as epididymitis, which is inflammation of the epididymis, the tube that carries sperm from the testicle. This condition can be caused by infections, including sexually transmitted infections, or even non-infectious causes such as trauma or prolonged sitting. Symptoms often include swelling, tenderness, and sometimes a fever.
Another possibility is testicular torsion, which is a medical emergency that occurs when the spermatic cord becomes twisted, cutting off the blood supply to the testicle. This condition typically presents with sudden, severe pain and swelling, and requires immediate medical attention. However, since you mentioned that the pain is intermittent and not constant, torsion may be less likely.
You also mentioned feeling small lumps in the scrotum. These could be benign cysts, such as epididymal cysts or spermatoceles, which are fluid-filled sacs that can develop in the epididymis. These are usually painless and do not require treatment unless they cause discomfort or grow significantly.
Your late-night study habits and lack of sleep could contribute to stress and muscle tension, which may indirectly lead to discomfort in the groin area. Stress can exacerbate pain perception and lead to muscle tightness, which might explain why you feel pain intermittently.
It’s crucial to monitor your symptoms closely. If the pain persists, worsens, or is accompanied by other symptoms such as swelling, redness, fever, or changes in urination, you should seek medical attention promptly. A healthcare provider may perform a physical examination, and possibly an ultrasound, to assess the testicles and surrounding structures for any abnormalities.
In summary, while intermittent testicular pain can stem from various causes, including benign conditions, it is essential to approach this symptom with caution. Maintaining good sleep hygiene, managing stress, and seeking medical advice if symptoms persist or worsen are important steps to take. Remember, your health is paramount, and it’s always better to err on the side of caution when it comes to symptoms involving the reproductive system.
