Red spots on the glans and itching in the urethra?

Hello, I recently engaged in anal intercourse and have noticed some red spots on the glans, which are only visible when erect (not noticeable in a flaccid state).
They are not painful or itchy.
Additionally, I have a sensation of itching inside the urethra, specifically around the glans area, which usually occurs after urination.
This itching sensation does not always require pressure to alleviate.
I visited a urologist, but I may not have communicated my symptoms clearly.
The doctor completed the examination quickly and prescribed a combination ointment (Yisilian), pain relief tablets (Limo Bole), and a coated tablet (You Ru Lin Lu Bi Jing).
However, it has been three to four days, and I feel like my condition has not improved.
Could you please advise me on what this situation might be? Also, how can I communicate more clearly with the urologist during my next visit?

A Cheng, 30~39 year old female. Ask Date: 2016/01/16

Dr. Xu Weikai reply Urology

A-Cheng: Hello, first of all, during your consultation, you must disclose any anal intercourse, as it may increase the risk of HIV infection.
The physician will typically focus on testing for sexually transmitted infections.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
Online responses should only be used as a reference; it is essential to seek medical care at a healthcare facility.

It sounds like you're experiencing some concerning symptoms related to your genital area, specifically red spots on the glans and itching in the urethra. Given your recent sexual activity, it's important to consider a few potential causes for these symptoms, as well as the best approach to communicate with your healthcare provider.

Firstly, the red spots on the glans that are only visible during an erection could be indicative of several conditions. These might include irritation or inflammation due to friction, a sexually transmitted infection (STI), or a dermatological condition such as balanitis, which is an inflammation of the glans. The fact that they are not painful or itchy at rest is somewhat reassuring, but it does not rule out the possibility of an underlying issue.

The itching sensation in the urethra, particularly after urination, could be related to several factors as well. It might be due to irritation from urine, a yeast infection, or again, an STI. Conditions like urethritis, which is inflammation of the urethra, can also cause similar symptoms.
You mentioned that you have already seen a urologist and were prescribed medications, including a topical cream and pain relief medication. If you have not noticed any improvement after several days of treatment, it may be time to follow up with your healthcare provider. When you do, it would be helpful to clearly describe your symptoms, including:
1. Duration and Onset: Explain when you first noticed the red spots and itching, and whether they have changed in appearance or intensity since they first appeared.

2. Associated Symptoms: Mention any other symptoms you may have experienced, such as discharge, pain during urination, or any changes in urinary habits.

3. Sexual History: Be open about your recent sexual activity, as this can help your doctor assess the risk of STIs or other infections.

4. Response to Treatment: Clearly state that you have been using the prescribed medications for several days without improvement. This is crucial information that can help your doctor decide on the next steps.

5. Concerns and Questions: Don't hesitate to express any concerns you have about the possibility of STIs or other conditions. Ask about further testing that may be necessary, such as urine tests or swabs, to rule out infections.

In addition to following up with your urologist, consider practicing safe sex in the future to reduce the risk of STIs. Using condoms can significantly lower the risk of transmission of infections during sexual activity.

Lastly, if you experience any worsening symptoms, such as increased pain, swelling, or fever, seek medical attention promptly. It's essential to address these symptoms early to prevent potential complications.

In summary, while your symptoms may not seem severe, they warrant further investigation, especially given your recent sexual history. Clear communication with your healthcare provider will be key in getting the appropriate diagnosis and treatment.
