Doctor, please help me with my questions?

Hello doctor, I originally had two or three red, swollen spots at the junction of my glans and foreskin that felt like pimples.
After applying medication (which was prescribed by a doctor for a similar condition in the past) for about one to two weeks, the swelling started to subside.
However, yesterday, the edges of those two spots suddenly became a bit red and appeared as a small red patch, and I noticed one or two additional small red dots.
What could be causing this? I would like to consult with you first! I only feel a stinging sensation when rinsing with water during my evening shower, as if the skin is broken when it comes into contact with water.
Other than that, there is no pain, and I don't feel any discomfort.

Xiao Xin, 20~29 year old female. Ask Date: 2016/01/19

Dr. Xu Weikai reply Urology

Xiao Xin: Hello, you may be experiencing localized inflammation, which is usually caused by excessive cleaning.
The recommended cleaning method for the glans and foreskin is as follows: 1.
Clean the glans several times a day.
2.
It is best to clean away any urine residue after each urination; rinsing is preferable, but if that's not convenient, simply patting dry is sufficient.
3.
Cleaning method: * use only cool, clean water .
Absolutely avoid using soap, body wash, or any cleaning agents.
If you feel dryness, you can apply petroleum jelly, baby oil, or a gentle, non-irritating moisturizer locally.
4.
If this occurs frequently, there may be other underlying issues, and it is advisable to seek professional medical advice.
If you have any further questions, feel free to ask, and we can discuss and research together.
Online responses should only be used as a reference; if necessary, it is still important to visit a medical facility.

More Info

Understanding red bumps on the penis, particularly at the junction of the glans (the head of the penis) and the foreskin, can be concerning for many individuals. The symptoms you describe, including the presence of red, swollen bumps that resemble pimples, and the recent increase in redness and the appearance of additional small red spots, warrant careful consideration and evaluation.

Possible Causes
1. Infection: One of the most common causes of red bumps in this area is an infection, which could be bacterial, fungal, or viral. Conditions such as balanitis (inflammation of the glans) or a sexually transmitted infection (STI) like herpes or HPV can present with similar symptoms. If the bumps are painful, itchy, or accompanied by discharge, this could indicate an infection.

2. Irritation or Allergic Reaction: Sometimes, irritation from soaps, lotions, or even laundry detergents can lead to localized inflammation and redness. Allergic reactions to personal care products or latex (from condoms) can also cause similar symptoms.

3. Pearly Penile Papules: These are small, harmless bumps that can appear around the corona of the glans. They are a normal anatomical variation and not a cause for concern.

4. Folliculitis: This is an inflammation of the hair follicles, which can occur in the genital area and may present as red bumps. It can be caused by friction, shaving, or bacterial infection.

5. Dermatological Conditions: Conditions such as psoriasis or eczema can also affect the genital area, leading to red, inflamed patches or bumps.

Recommended Actions
1. Avoid Irritants: Until you can see a healthcare provider, avoid using any potential irritants, including scented soaps or lotions. Stick to gentle, unscented cleansers.

2. Keep the Area Clean and Dry: Maintain good hygiene by washing the area with mild soap and water, and ensure it is thoroughly dried afterward.

3. Monitor Symptoms: Keep an eye on the bumps. If they continue to grow, become painful, or if you develop additional symptoms such as fever, discharge, or systemic symptoms, seek medical attention promptly.

4. Consult a Healthcare Provider: Given that you have already used a topical medication prescribed for a similar condition, it’s essential to follow up with your healthcare provider. They may want to perform a physical examination and possibly run tests to determine the exact cause of your symptoms. This could include swabs for infections or a biopsy if there is concern about a dermatological condition.

5. Avoid Self-Medication: While it may be tempting to apply over-the-counter treatments, it’s best to avoid self-medication without a proper diagnosis. Some treatments may exacerbate the condition or lead to further irritation.

Conclusion
The appearance of red bumps on the penis can stem from various causes, ranging from benign to more serious conditions. Given the changes you've observed, including increased redness and the emergence of new bumps, it is crucial to consult with a healthcare professional for an accurate diagnosis and appropriate treatment. Early intervention can help prevent potential complications and provide peace of mind.
