Penile Adhesions: Is Regular Cleaning Enough? - Urology

The foreskin retracts completely, but the tip of the glans on the left side is adhered to the foreskin?

My foreskin can be pulled back freely, and it is not a problem of phimosis or excessive foreskin as mentioned online.
When I fully retract the foreskin, there is a small section on the left side where the foreskin is adhered to the tip of the glans, but it doesn't seem to be smegma as described online.
Should I just regularly retract it during bathing for it to separate?

GFM, 10~19 year old female. Ask Date: 2016/01/21

Dr. Xu Weikai reply Urology

GFM: When cleaning, gently pull apart to gradually separate.
The recommended method for cleaning the glans and foreskin is as follows: 1.
Clean the glans several times a day.
2.
It is best to clean away any smegma after each urination; if inconvenient, simply pat the area dry.
3.
Cleaning method: * use only cool, clean water .
Absolutely do not use soap, body wash, or any cleaning agents.
If dryness occurs, you may apply a small amount of petroleum jelly, baby oil, or a mild, non-irritating moisturizer.
4.
If issues frequently recur, there may be other underlying problems, and it is advisable to seek professional medical advice.
If there are any unclear points, feel free to ask again for further discussion and research.
Online responses should only serve as a reference; if necessary, it is important to visit a medical facility.

Understanding penile adhesions, particularly in the context of the foreskin and glans, is important for maintaining genital health. Penile adhesions can occur when the foreskin adheres to the glans (the head of the penis), which can be a normal anatomical variation, especially in younger males. However, in adults, this can sometimes lead to discomfort or hygiene issues.

From your description, it seems that you have a small area where the foreskin is adhered to the glans. This is not uncommon, and it is often not a cause for concern unless it leads to pain, inflammation, or difficulty in hygiene. Regular cleaning is essential for preventing infections and maintaining overall genital health. Here are some key points to consider regarding your situation:
1. Hygiene Practices: It is crucial to maintain proper hygiene in the genital area. When you shower, gently retract the foreskin to clean the area underneath. Use mild soap and warm water to wash the glans and the inner foreskin. Avoid harsh soaps or chemicals, as these can irritate the sensitive skin in this area.

2. Regular Retraction: If the foreskin can be retracted without pain, doing so regularly during bathing can help prevent adhesions from forming or worsening. Over time, this may help the adhered area to separate naturally. However, do this gently to avoid causing any trauma or irritation.

3. Signs of Infection or Complications: Be vigilant for any signs of infection, such as redness, swelling, discharge, or pain. If you notice any of these symptoms, it is essential to consult a healthcare provider. Infections can lead to more severe complications if left untreated.

4. Consultation with a Specialist: If the adhesion persists or causes discomfort, it may be worth consulting a urologist. They can assess the situation more thoroughly and provide recommendations. In some cases, minor surgical intervention may be necessary to separate the adhered skin, especially if it is causing significant issues.

5. Avoiding Irritation: Be cautious about any activities that may irritate the area, such as vigorous cleaning or sexual activity, until you are sure that the adhesion is not causing any problems.
6. Education on Normal Anatomy: Understanding that some degree of adhesion can be normal is important. Many men have some degree of adherence between the foreskin and glans, and it does not always require intervention.
In summary, regular cleaning and gentle retraction of the foreskin during bathing are generally sufficient to maintain hygiene and potentially resolve minor adhesions. However, if you have concerns about pain, discomfort, or persistent adhesion, seeking medical advice is always a prudent step. Your healthcare provider can offer personalized guidance based on your specific situation and ensure that your genital health is well-managed.
