Shoulder pain (is it tendonitis?)
Hello Doctor: After working for a while, I noticed some discomfort in my shoulder, which then turned into soreness and later became painful! The pain is so bad that I can't sleep well at night.
( ´▽` )ノ Lifting my arm causes pain in my shoulder, moving it backward also hurts, and rotating it is painful too.
The only movement that doesn't hurt is extending my arm straight forward; any other movement causes pain.
Which specialty should I see? Orthopedics? Rehabilitation? Neurosurgery? Thank you, Doctor ^_^ I feel like my arms and limbs are not my own.

Dr. Shi Guozheng reply Orthopedics
It may be tendonitis; you can start with heat therapy or take some pain relievers and then see an orthopedic specialist.

More Info
Shoulder pain can be a complex issue, often stemming from various underlying conditions, including tendonitis, rotator cuff injuries, bursitis, or even referred pain from cervical spine issues. Based on your description, it sounds like you are experiencing significant discomfort that affects your daily activities and sleep, which is understandably concerning.
Understanding Your Symptoms
1. Pain Characteristics: You mentioned that the pain worsens with movement, especially when lifting your arm or rotating it. This is a common symptom of shoulder tendonitis or a rotator cuff injury. Tendonitis typically presents as pain and tenderness in the shoulder, especially during overhead activities or when reaching behind your back.
2. Sleep Disturbance: The fact that your pain disrupts your sleep is significant. Many patients with shoulder issues find it difficult to sleep on the affected side, and this can lead to further discomfort and a cycle of pain and sleep deprivation.
3. Limited Range of Motion: The inability to move your arm freely without pain suggests that there may be inflammation or injury to the rotator cuff or surrounding structures. If you can move your arm forward without pain, but experience pain with other movements, it may indicate specific tendon involvement.
Recommended Actions
Given your symptoms, here are some steps you can take:
1. Consult a Specialist: Since your symptoms are affecting your quality of life, it would be wise to consult an orthopedic specialist or a physical medicine and rehabilitation (PM&R) physician. An orthopedic doctor specializes in musculoskeletal issues, including shoulder injuries, while a PM&R physician can provide a comprehensive approach to rehabilitation.
2. Imaging Studies: Your doctor may recommend imaging studies, such as an MRI or ultrasound, to assess the condition of your rotator cuff and other soft tissues around the shoulder. This can help in diagnosing conditions like tendonitis, tears, or bursitis.
3. Physical Therapy: Regardless of the specific diagnosis, physical therapy is often beneficial for shoulder pain. A physical therapist can design a rehabilitation program that includes stretching and strengthening exercises tailored to your condition. This can help improve your range of motion and reduce pain.
4. Medication: Over-the-counter anti-inflammatory medications (like ibuprofen or naproxen) can help reduce inflammation and alleviate pain. If your pain is severe, your doctor may prescribe stronger medications or corticosteroid injections to provide relief.
5. Activity Modification: It’s essential to avoid activities that exacerbate your pain. Modifying your daily activities and avoiding overhead movements can help prevent further irritation of the shoulder.
6. Alternative Treatments: Some patients find relief through alternative treatments such as acupuncture, massage therapy, or chiropractic care. These can be considered as adjunct therapies alongside conventional treatments.
Conclusion
Shoulder pain can significantly impact your daily life, and it’s crucial to address it promptly. By consulting with the appropriate specialists, undergoing necessary imaging, and engaging in a structured rehabilitation program, you can work towards alleviating your pain and restoring function to your shoulder. Remember, early intervention often leads to better outcomes, so don’t hesitate to seek help.
