White mucous stools with blood?

Hello Doctor, I would like to ask about something.
Recently, I have noticed white mucus in my stool, which has made me a bit worried.
I consulted at the nearest hospital and underwent a stool occult blood test, which came back negative.
The doctor mentioned it might be hemorrhoids and prescribed some topical medication for me.
However, after a few days, I experienced fresh blood during bowel movements along with some pain, particularly at the beginning and end of the stool, which is bright red.
This has led me to wonder if it could be an issue with internal hemorrhoids.
Also, what could the white mucus indicate? Thank you, Doctor.

Dr. Cai Anshun reply Gastroenterology and Hepatology

Hello: It is likely a normal phenomenon of colonic mucosal shedding.
However, if there is persistent or increasing fresh blood in the stool, it is advisable to return for a follow-up with a gastroenterologist or a colorectal specialist.

Understanding the presence of white mucus in stool can be concerning for many individuals, and it is essential to approach this symptom with a comprehensive understanding of potential causes and implications. The presence of white mucus in the stool can arise from various factors, and while it is often benign, it can sometimes indicate underlying health issues.

Mucus is a normal component of the digestive system, produced by the intestines to help lubricate the stool and facilitate its passage through the gastrointestinal tract. However, when there is an increase in mucus production or a noticeable change in its appearance, it can raise questions about gastrointestinal health.

In your case, the presence of white, sticky mucus in your stool, combined with symptoms such as fresh blood and pain during bowel movements, suggests a few possibilities. One common cause of increased mucus production is irritation or inflammation in the intestines. Conditions such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), or infections can lead to increased mucus secretion. In IBS, for example, the intestines may become overly sensitive, leading to changes in bowel habits and increased mucus production.

Another possibility is the presence of hemorrhoids, which you mentioned your doctor suspected. Hemorrhoids can cause discomfort and bleeding, particularly during bowel movements. The irritation from hemorrhoids can also lead to increased mucus production as the body attempts to protect the affected area. The fresh red blood you observe may indeed be from hemorrhoids, especially if it is located at the beginning or end of your stool.

The white mucus itself may not be a direct cause for concern, particularly if it is not accompanied by other alarming symptoms such as significant abdominal pain, persistent diarrhea, or weight loss. However, it is essential to monitor your symptoms closely. If the mucus persists, or if you experience worsening symptoms, further evaluation may be warranted.

In terms of treatment, addressing the underlying cause is crucial. If hemorrhoids are confirmed, your doctor may recommend topical treatments, dietary changes to increase fiber intake, and adequate hydration to soften stools and reduce straining during bowel movements. This can help alleviate both the hemorrhoids and the associated symptoms.

If the mucus continues to be a concern, or if you develop additional symptoms such as changes in bowel habits, abdominal pain, or unexplained weight loss, it would be prudent to follow up with your healthcare provider. They may consider additional diagnostic tests, such as a colonoscopy, to rule out more serious conditions, including colorectal cancer or inflammatory bowel diseases.

In summary, while the presence of white mucus in your stool can be concerning, it is often a benign symptom related to irritation or inflammation in the gastrointestinal tract. Given your symptoms of fresh blood and pain, it is essential to address these issues with your healthcare provider to ensure proper diagnosis and treatment. Maintaining open communication with your doctor and following their recommendations will be key to managing your symptoms effectively.
