Snoring during sleep, followed by waking up due to airway obstruction?

Hello Dr.
Zheng, my girlfriend is 30 years old and works a rotating shift schedule (24 hours).
About every two months, she has to work a night shift.
When she sleeps during the day, I am next to her.
She usually has sleep disturbances and tends to wake up frequently.
I’ve noticed that when she is in deep sleep, she snores, and then her nose gradually becomes congested, after which she seems to wake up.
Then she goes back to deep sleep and snores again, followed by nasal congestion, and it feels like she wakes up again.
This cycle continues.
I would like to ask, doctor, is her poor sleep related to her nasal issues? She already has a history of nasal allergies (she experiences allergies in cold weather, sneezing, and difficulty breathing).
I have two questions for you: 1.
What kind of symptoms is she experiencing? 2.
What solutions are available? Thank you for your patient response, doctor.

Hello: Snoring may be related to narrowing of the nasal passages or the oropharynx.
If you experience daytime sleepiness or snoring accompanied by sleep apnea, it is recommended to consult an ENT specialist or a pulmonologist at a hospital.
Wishing you safety and health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the ENT Department, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

Hello! Thank you for your questions regarding your girlfriend's sleep issues, particularly in relation to her snoring and nasal congestion. Based on the symptoms you've described, it seems that she may be experiencing a combination of sleep apnea and nasal obstruction, which can significantly impact her sleep quality.

Understanding the Symptoms
1. Snoring and Nasal Congestion: Snoring is often associated with obstructive sleep apnea (OSA), a condition where the airway becomes partially or completely blocked during sleep, leading to disrupted breathing. The fact that your girlfriend experiences nasal congestion, especially during sleep, suggests that her nasal passages may be contributing to her snoring and sleep disturbances. Nasal congestion can be caused by allergies, which you mentioned she has, or anatomical issues such as enlarged turbinates (nasal structures that can swell) or a deviated septum.

2. Sleep Fragmentation: The pattern of waking up after periods of deep sleep, followed by snoring and nasal blockage, indicates that her sleep is being interrupted. This can lead to excessive daytime sleepiness, fatigue, and difficulty concentrating, as her body is not getting the restorative sleep it needs.

Possible Causes
- Allergic Rhinitis: Given her history of nasal allergies, this could be a significant factor. Allergies can cause inflammation and swelling in the nasal passages, leading to congestion and difficulty breathing during sleep.

- Obstructive Sleep Apnea: If her airway is obstructed due to her sleeping position, nasal congestion, or other factors, this could lead to episodes of apnea (pauses in breathing) that cause her to wake up frequently.

- Circadian Rhythm Disruption: Working rotating shifts can disrupt the body's natural circadian rhythm, making it harder for her to achieve consistent, quality sleep.

Solutions and Recommendations
1. Consultation with a Specialist: It would be beneficial for her to see an Ear, Nose, and Throat (ENT) specialist to evaluate her nasal passages and determine if there are any anatomical issues contributing to her symptoms. A sleep study may also be recommended to assess for sleep apnea.

2. Allergy Management: If allergies are a contributing factor, managing them with antihistamines or nasal corticosteroids may help reduce nasal congestion. Keeping her sleeping environment clean and free from allergens (like dust mites and pet dander) can also be beneficial.

3. Nasal Irrigation: Using a saline nasal spray or a neti pot can help clear nasal passages and reduce congestion, especially before bedtime.

4. Sleep Hygiene Practices: Encourage her to establish a regular sleep schedule, even with her rotating shifts. This includes creating a dark, quiet, and cool sleeping environment, which can help improve sleep quality.

5. Positioning: If she tends to sleep on her back, encourage her to try sleeping on her side. This position can help reduce snoring and the likelihood of airway obstruction.

6. Weight Management: If applicable, maintaining a healthy weight can reduce the severity of sleep apnea symptoms.

7. CPAP Therapy: If diagnosed with sleep apnea, Continuous Positive Airway Pressure (CPAP) therapy may be recommended to keep her airway open during sleep.

Conclusion
In summary, your girlfriend's symptoms of snoring, nasal congestion, and sleep disturbances could be related to both her nasal allergies and potential sleep apnea. Addressing these issues through medical evaluation, allergy management, and good sleep hygiene practices can significantly improve her sleep quality. Encourage her to seek professional help to explore these options further. Thank you for your inquiry, and I hope this information helps both of you find a path to better sleep!
