Inverted nipple?

Hello, doctor.
I am currently 19 years old and have an issue with inverted nipples, which I first noticed during breast development in elementary school.
However, I have not addressed it (I visited the hospital twice, but they advised me to wait and observe until I grow older, suggesting it could be dealt with later).
I feel that the inversion is quite severe, and I read online that if it’s not serious, I could try to pull out the nipple daily to help it maintain its shape, but in my case, it has become impossible to pull out! Sometimes, the inverted nipple itches, and the skin around the nipple is very thin and delicate, with a light pink color.
Is it correct to clean the secretions trapped inside daily? Also, is using a nipple correction device feasible? (I’m not sure if it’s specifically for use before pregnancy.) Thank you.

Xiao Wen, 10~19 year old female. Ask Date: 2016/01/28

Hello! With the presence of discharge and inversion, the first step is to rule out the possibility of a breast tumor.
Although you mentioned, "I first noticed it when my breasts were developing in elementary school," and "my condition has progressed to a stage where it cannot be pulled out," indicating a third-degree inversion.
Typically, inversion occurs during the postpartum and breastfeeding periods.
However, congenital inversion is reported in the literature to range from 10% to 30%.
Generally, it does not affect breastfeeding but only impacts appearance.
Nipple correction devices such as Supple Cups and breast shells are available.
These devices are designed to help the tissue beneath the nipple gradually relax, and cleaning out the accumulated discharge daily can help prevent dirt buildup.
If these methods are ineffective, surgical intervention may need to be considered, although this could result in the loss of lactation ability.
Changhua Hospital cares about you!

More Info

Inverted nipples, or nipple inversion, is a condition where the nipple is retracted into the breast rather than protruding outward. This can occur in one or both breasts and can be present from birth or develop later in life. For a 19-year-old like yourself, it’s understandable to have concerns about this condition, especially if it has been persistent since puberty.

Firstly, it’s important to note that inverted nipples are generally not a medical emergency and often do not pose any health risks. However, they can sometimes be associated with underlying conditions, so it’s good that you’ve consulted with healthcare professionals in the past. The recommendation to observe and wait until you are older is common, as many changes in breast tissue occur during adolescence and early adulthood.

Regarding your concerns about hygiene, it is indeed advisable to keep the area clean. If you notice any discharge or secretions, gently cleaning the area can help prevent irritation or infection. However, be cautious not to irritate the delicate skin around the nipple, especially since you mentioned that the skin is thin and sensitive.

As for the use of nipple correction devices, these can be effective for some individuals. Nipple correction devices, or nipple pullers, are designed to help draw out inverted nipples and can be used to encourage the nipple to protrude. While they are often marketed for use during pregnancy or breastfeeding, they can also be used by individuals who are not pregnant. However, it’s important to use these devices as directed and to consult with a healthcare provider before starting any new treatment. They can provide guidance on the appropriate use of these devices and help you understand what to expect.

If you find that the inversion is causing discomfort, such as itching or sensitivity, it may be worth discussing further treatment options with a specialist. Some individuals choose surgical options if the inversion is severe or if it causes significant psychological distress. Surgical procedures can involve techniques to release the tissue that causes the nipple to retract, allowing it to protrude more naturally.

In summary, while inverted nipples are often benign, your concerns are valid, especially regarding comfort and aesthetics. Maintaining proper hygiene is essential, and using a nipple correction device may be beneficial. However, consulting with a healthcare provider, preferably a specialist in breast health, can provide you with tailored advice and treatment options based on your specific situation. They can help you weigh the pros and cons of various approaches and determine the best course of action for your needs.
