Erectile dysfunction?

I recently started a sexual relationship with my girlfriend, but during intercourse, my penis becomes soft.
I don't feel much during the act, but it can become hard again with manual or oral stimulation.
However, it softens shortly after penetration.
I have normal erections when I masturbate.
I had a circumcision in middle school, and I've read online that circumcised individuals may have reduced sensitivity.
Is this true? I am 173 cm tall and weigh 112 lbs, and I often work late nights.
Thank you for your response.

Pangzi, 30~39 year old female. Ask Date: 2016/02/02

Obese individual: Hello, it is possible that there is insufficient stimulation, which may be due to limitations in posture or body type, resulting in inadequate intensity of stimulation, and consequently, this can affect erection.
Additionally, it may also be related to poor cardiopulmonary function.
I recommend regular exercise: at least 3-5 times a week, with each session lasting over 40-60 minutes, and the intensity should be sufficient.
If you have further questions, feel free to ask again for more discussion and research.
Please note that online responses should only serve as a reference; it is essential to seek medical attention at a healthcare facility if necessary.

Erectile dysfunction (ED) is a common issue that many men face, especially when they are in new sexual relationships. Your experience of having difficulty maintaining an erection during intercourse, while still being able to achieve an erection through manual or oral stimulation, is not uncommon. There are several potential causes for this issue, and understanding them can help you find appropriate solutions.

Psychological Factors
One of the most significant contributors to erectile dysfunction, especially in younger men, is psychological factors. Anxiety, stress, and performance pressure can all lead to difficulties in maintaining an erection. Since you mentioned that you recently started a sexual relationship, it’s possible that nerves or anxiety about performance are affecting your ability to maintain an erection. This is often referred to as "performance anxiety."
Additionally, if you have any underlying concerns about your sexual performance or body image, these can also contribute to ED. It’s important to communicate openly with your partner about your feelings and experiences, as this can help alleviate some of the pressure you may be feeling.

Physical Factors
While psychological factors are often at play, there can also be physical causes of erectile dysfunction. You mentioned that you had a circumcision in middle school, and there is some debate in the medical community about whether circumcision affects sensitivity. Some studies suggest that circumcision may lead to reduced sensitivity due to the removal of sensitive skin, while others indicate that it does not significantly impact sexual function.
Your lifestyle choices can also play a role in erectile function. You noted that you often work late and may not be getting enough sleep. Fatigue, poor sleep quality, and irregular sleep patterns can negatively affect hormone levels and sexual function. Additionally, factors such as diet, exercise, and overall health can influence erectile performance. Maintaining a healthy weight, exercising regularly, and eating a balanced diet can all contribute to better sexual health.

Hormonal Factors
Hormonal imbalances can also lead to erectile dysfunction. Testosterone is a key hormone in male sexual function, and low levels can result in difficulties with erections. If you suspect that hormonal issues may be a factor, it may be worth discussing this with a healthcare provider who can evaluate your hormone levels.

Solutions and Recommendations
1. Open Communication: Talk to your partner about your experiences and feelings. This can help reduce anxiety and create a more supportive environment.

2. Consult a Healthcare Professional: If the issue persists, consider consulting a urologist or a healthcare provider specializing in sexual health. They can help identify any underlying physical or hormonal issues and recommend appropriate treatments.

3. Lifestyle Changes: Focus on improving your overall health. Ensure you are getting enough sleep, eating a balanced diet, and engaging in regular physical activity. Reducing alcohol consumption and quitting smoking can also improve erectile function.

4. Stress Management: Practice relaxation techniques such as deep breathing, meditation, or yoga to help manage anxiety and stress.

5. Therapy: If psychological factors are contributing to your erectile dysfunction, consider speaking with a therapist or counselor who specializes in sexual health. Cognitive-behavioral therapy (CBT) can be particularly effective in addressing performance anxiety.

6. Medication: If lifestyle changes and therapy do not resolve the issue, medications such as phosphodiesterase type 5 inhibitors (e.g., Viagra, Cialis) may be prescribed by a healthcare provider to help improve erectile function.

In conclusion, erectile dysfunction can be caused by a combination of psychological and physical factors. By addressing these issues through open communication, lifestyle changes, and professional guidance, you can work towards improving your sexual health and overall well-being. Remember, you are not alone in this experience, and many men face similar challenges. Seeking help is a positive step towards finding a solution.
