Cardiac issues
Hello doctor, I have been feeling like there is something wrong with my heart for the past six months.
My symptoms include occasional shortness of breath while sleeping, and I feel a sense of pressure in my heart when lying on my left or right side, which can even lead to difficulty breathing.
As a result, I can only sleep in a semi-side or flat position to minimize discomfort.
After intense exercise, I experience a rapid heartbeat and significant discomfort, such as when running, but I generally feel fine during regular work activities.
Could you please advise me on what type of issue this might be? Two months ago, during my military physical examination, the doctor recommended that I go to a large hospital for further testing, but I have not done so yet.
I wanted to consult with you first to understand what kind of problems might be involved.
Thank you for your help.

Hello, based on your description, there is a higher likelihood of autonomic nervous system dysfunction.
However, it is difficult to determine if there are any organic issues.
It is recommended that you visit a cardiology clinic for further examination.

When it comes to understanding heart issues, it's crucial to recognize the symptoms and know when to seek medical help. Based on your description, it seems you are experiencing significant discomfort related to your heart, particularly during sleep and after physical exertion. Symptoms such as shortness of breath, a feeling of pressure in the chest, and difficulty breathing when lying down can be concerning and warrant further investigation.
Firstly, it's important to understand that the heart is a vital organ responsible for pumping blood throughout the body. Any symptoms that suggest it is not functioning properly should be taken seriously. The symptoms you describe, especially the feeling of pressure in your chest and difficulty breathing, could indicate several potential issues, ranging from anxiety-related symptoms to more serious cardiac conditions.
One possibility is that you may be experiencing anxiety or panic attacks, which can manifest as physical symptoms, including chest tightness and shortness of breath. However, given your symptoms occur during sleep and after intense physical activity, it is essential to consider cardiac causes as well. Conditions such as angina (chest pain due to reduced blood flow to the heart), arrhythmias (irregular heartbeats), or even structural heart issues could be at play.
The fact that you feel more comfortable in a semi-reclined position suggests that there may be some underlying issue that affects your heart's ability to function optimally when you are lying flat. This could be related to fluid retention or pressure on the heart, which can occur in various cardiac conditions. Additionally, experiencing a rapid heartbeat after vigorous exercise is not uncommon, but if it is accompanied by discomfort or other symptoms, it may require further evaluation.
Given that you have already been advised by a physician to seek further examination at a larger hospital, it would be prudent to follow through with that recommendation. Diagnostic tests such as an electrocardiogram (ECG), echocardiogram, or stress tests can provide valuable insights into your heart's health. These tests can help identify any abnormalities in heart rhythm, structure, or function that may be contributing to your symptoms.
In terms of when to seek help, you should not hesitate to contact a healthcare provider if you experience any of the following:
1. Persistent Chest Pain: If the pressure or pain in your chest persists or worsens, it is critical to seek immediate medical attention.
2. Shortness of Breath: If you find it increasingly difficult to breathe, especially at rest or with minimal exertion, this is a red flag.
3. Dizziness or Fainting: Experiencing lightheadedness, dizziness, or fainting spells can indicate a serious issue that requires prompt evaluation.
4. Rapid or Irregular Heartbeat: If your heart races or beats irregularly, especially if accompanied by other symptoms, it is important to get checked out.
In conclusion, while anxiety can certainly contribute to physical symptoms, the nature of your symptoms suggests that a thorough cardiac evaluation is warranted. Don't delay in seeking the necessary tests and consultations. Early detection and intervention can be crucial in managing any potential heart issues effectively. Remember, your health is paramount, and addressing these symptoms with a healthcare professional is the best course of action.
