A white bump on the vulva that itches?
Recently, I developed a small white bump at the vaginal opening, about the size of a grain of rice.
It has been there for approximately two weeks and is itchy.
It causes pain during sexual intercourse due to friction, but it hasn't increased in size or number during these two weeks.
However, in the past few days, the entire genital area, including the anal region, has been itchy, although I haven't noticed similar white bumps elsewhere.
The itching is quite bothersome, and I have also noticed an increase in clear discharge with no particular odor.
I would like to know what this issue might be and whether it is serious.
Thank you for your patient response, doctor.

The presence of a white bump on the vulva, especially one that is itchy and painful during intercourse, can be concerning and warrants careful consideration. Based on your description, there are several potential causes for this symptom, and it is important to approach this issue with a thorough understanding of possible conditions.
One common cause of a white bump in the vulvar area is a sebaceous cyst or epidermoid cyst, which can appear as small, painless lumps. However, if the bump is itchy and painful, it might suggest a different issue, such as a folliculitis (inflammation of hair follicles) or a bartholinitis (inflammation of the Bartholin's glands), which can lead to painful cysts.
Another possibility is a vulvar dermatological condition, such as contact dermatitis, which can occur due to irritation from soaps, detergents, or even certain fabrics. This can lead to itching and discomfort, particularly during sexual activity. Additionally, conditions like vulvar lichen sclerosus or lichen planus can cause white patches or bumps and are often associated with itching and pain.
Infections are also a significant consideration. Yeast infections (candidiasis) can cause intense itching and discomfort, and while they typically present with a thick, white discharge, they can sometimes lead to the formation of small bumps. Sexually transmitted infections (STIs), such as herpes or human papillomavirus (HPV), can also manifest as bumps or lesions in the genital area and may be accompanied by itching or pain.
Given that you have experienced increased itching in the vulvar and anal areas, along with changes in discharge, it is crucial to consider the possibility of a yeast infection or another type of infection. The transparent discharge you mentioned, especially if it is odorless, could indicate a non-infectious cause, but it is essential to rule out infections.
In terms of severity, while some conditions may resolve on their own or with over-the-counter treatments, others may require medical intervention. For example, if the bump is due to an infection or a sexually transmitted disease, appropriate treatment is necessary to prevent complications and alleviate symptoms.
It is highly recommended that you consult a healthcare provider, preferably a gynecologist, for a thorough examination. They can perform a physical examination, possibly including a Pap smear or cultures, to determine the exact nature of the bump and any associated symptoms. Based on the diagnosis, they can provide appropriate treatment options, which may include topical medications, antifungal treatments, or other therapies as needed.
In the meantime, to alleviate discomfort, consider avoiding irritants such as scented soaps or lotions, wearing breathable cotton underwear, and maintaining good hygiene. Over-the-counter antihistamines may help with itching, but it is best to seek professional advice for a tailored approach to your symptoms.
In summary, while the white bump and associated symptoms could be due to various benign conditions, it is essential to seek medical evaluation to ensure proper diagnosis and treatment. Early intervention can help prevent potential complications and improve your quality of life.
