Severe itching of the foreskin?
Hello Doctor: Recently, I've been experiencing intense itching on the inner side of my foreskin and the glans, which leads me to rub the area uncontrollably.
After rubbing, it becomes red and swollen, resembling edema.
I do not have a problem with phimosis, as my foreskin can fully retract to expose the glans.
This same condition occurred two years ago, but after visiting a doctor and using a topical ointment for a few days, it resolved.
What can be done to address this recurring infection, or are there other solutions available? Thank you, Doctor.

Hello, it is likely not a recurrent infection.
It may be due to excessive cleaning of the glans and foreskin.

More Info
Persistent itching under the foreskin can be a frustrating and uncomfortable issue, and it’s essential to understand the potential causes and appropriate solutions. The symptoms you describe—intense itching, redness, and swelling—can be indicative of several underlying conditions.
Possible Causes:
1. Infections: One of the most common causes of itching in the genital area is an infection. This could be a yeast infection (Candida), bacterial infection, or sexually transmitted infections (STIs) such as herpes or trichomoniasis. Yeast infections, in particular, are known to cause significant itching and irritation.
2. Dermatitis: Contact dermatitis can occur due to irritation from soaps, detergents, or personal hygiene products. Allergic reactions to these substances can lead to inflammation and itching.
3. Poor Hygiene: Inadequate cleaning under the foreskin can lead to the accumulation of smegma, a substance that can cause irritation and itching. Proper hygiene is crucial in preventing such issues.
4. Skin Conditions: Conditions like psoriasis or eczema can also affect the genital area, leading to itching and discomfort. These conditions may require specific treatments to manage effectively.
5. Phimosis: Although you mentioned that you do not have a problem with phimosis, it is worth noting that tight foreskin can lead to irritation and difficulty in cleaning, which can exacerbate itching.
Solutions:
1. Consult a Healthcare Provider: Since you have experienced this issue repeatedly, it is crucial to consult a healthcare provider for a thorough examination. They may perform tests to identify any infections or skin conditions that need treatment.
2. Topical Treatments: If an infection is diagnosed, your doctor may prescribe antifungal or antibacterial creams. Over-the-counter hydrocortisone cream can help reduce inflammation and itching temporarily, but it should not be used long-term without medical advice.
3. Maintain Hygiene: Regularly clean the area with mild soap and water. Ensure that you dry the area thoroughly after washing, as moisture can promote fungal growth.
4. Avoid Irritants: Identify and avoid any products that may cause irritation. This includes scented soaps, lotions, or laundry detergents. Opt for hypoallergenic products designed for sensitive skin.
5. Loose-fitting Clothing: Wearing loose-fitting underwear and clothing can help reduce friction and irritation in the genital area.
6. Diet and Lifestyle: Sometimes, dietary changes can help manage yeast infections. Reducing sugar intake and incorporating probiotics may help maintain a healthy balance of bacteria and yeast in the body.
7. Follow-up Care: If the itching persists despite treatment, follow up with your healthcare provider. They may refer you to a dermatologist or urologist for further evaluation.
Conclusion:
Persistent itching under the foreskin can stem from various causes, including infections, dermatitis, and hygiene issues. It is essential to seek medical advice for a proper diagnosis and treatment plan. While topical treatments and improved hygiene can provide relief, addressing the underlying cause is crucial for long-term resolution. Regular follow-up with your healthcare provider will ensure that any recurring issues are managed effectively. Remember, open communication with your healthcare provider about your symptoms and concerns is key to finding the right solution.
