Causes of Left Lower Abdominal Pain

Hello Doctor: In June 2015, I had an abnormal result from a Pap smear, and after a follow-up examination, the doctor diagnosed me with cervicitis and prescribed a suppository.
However, starting in July of the same year, I began to experience brown discharge whenever my menstrual cycle was approaching (which I had never experienced before).
Since September, I have occasionally felt a dull pain in my lower left abdomen (which is bearable), and it has now progressed to sharp pains that occur daily.
Last week, I underwent a colonoscopy, which showed no issues.
I visited a gynecologist who performed a colposcopy and mentioned that my left ovary is slightly enlarged, but the degree of enlargement is within a tolerable range, and the doctor prescribed medication for muscle spasms.
Could this be an ovarian cyst? However, the pain in my lower left abdomen has not improved, and I still do not understand why I continue to experience this dull pain.
I am feeling anxious because I will be studying abroad in July of this year, and I have not been able to find the cause...
Thank you, Doctor.

Hello, the causes of abdominal pain in women can be numerous, potentially involving issues related to gastroenterology, obstetrics and gynecology, urology, or colorectal surgery, among others.
If a diagnosis has not yet been made, it is advisable to discuss with your physician and inquire about which specialist should be consulted next.

Understanding left lower abdominal pain can be complex, especially when it persists over time and is accompanied by other symptoms. Based on your description, several potential causes could be contributing to your discomfort, and it's essential to consider both physical and psychological factors.

Firstly, the history of cervical inflammation and the recent findings of an enlarged left ovary could suggest a few possibilities. One common condition that might be related is an ovarian cyst. Ovarian cysts are fluid-filled sacs that can develop on the ovaries and are often benign. They can cause discomfort, especially if they become large or rupture. The fact that your gynecologist noted the enlargement of the left ovary and prescribed medication for muscle spasms indicates that they are considering this possibility. However, not all ovarian cysts cause pain, and the degree of enlargement can vary widely without causing significant symptoms.

Another potential cause of your left lower abdominal pain could be related to gastrointestinal issues. While you mentioned that a colonoscopy showed no problems, conditions such as irritable bowel syndrome (IBS) or diverticulitis could still be considered. IBS can cause chronic abdominal pain and changes in bowel habits, while diverticulitis can lead to localized pain, particularly in the lower left abdomen.
Additionally, the psychological aspect of your situation cannot be overlooked. Anxiety and stress can exacerbate physical symptoms, including abdominal pain. The anticipation of your upcoming move for studies and the uncertainty surrounding your health can contribute to heightened anxiety, which may manifest as physical discomfort. It's not uncommon for individuals with anxiety to experience somatic symptoms, including abdominal pain, which can create a cycle of worry and discomfort.

Given your ongoing symptoms, it is crucial to continue working closely with your healthcare providers. If the pain persists despite the current treatment, further evaluation may be warranted. This could include imaging studies like an ultrasound or MRI to assess the ovaries and surrounding structures more closely. Additionally, consulting with a gastroenterologist may provide further insights into any potential gastrointestinal issues that could be contributing to your symptoms.

In the meantime, managing anxiety through relaxation techniques, counseling, or cognitive-behavioral therapy may also be beneficial. Addressing the psychological component of your pain can sometimes lead to significant improvements in physical symptoms.

In summary, while your left lower abdominal pain could be related to ovarian issues, gastrointestinal conditions, or even anxiety, a thorough evaluation and open communication with your healthcare team are essential for determining the underlying cause and appropriate treatment. Don't hesitate to seek a second opinion if you feel your concerns are not being adequately addressed. Your health and well-being are paramount, especially as you prepare for this exciting new chapter in your life.
