Menstrual delay

Hello Doctor, I would like to ask about my girlfriend's menstrual cycle.
Her period is already delayed by almost 10 days; it was expected to start around February 1st, but as of now, February 9th, it still hasn't arrived (it has been around the same time for the past two months).
We had sexual intercourse on January 31st, but we used a condom the entire time.
Additionally, she was hospitalized for 10 days due to APN (acute pyelonephritis) and received intravenous antibiotics.
However, since there was no improvement, she was switched to a stronger antibiotic.
After being discharged on January 27th, she continued taking antibiotic capsules for another 5 days.
I would like to know if this could be the reason for her period being significantly delayed.
I'm quite worried about the possibility of pregnancy since it still hasn't come.
Thank you, Doctor.

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the questioner: Women often experience hormonal imbalances affecting their menstrual cycle due to physiological and psychological stress after illness.
Antibiotics may also have an impact.
Other factors such as environment, lifestyle, emotions, diet, and age can also contribute.
It is advisable to consult a physician if your period is more than a week late to determine whether it is due to hormonal imbalance or early pregnancy.
Thank you! Wishing you peace and health, Dr.
Zhang.

Delayed menstruation can be a source of concern for many women, especially when it deviates from their regular cycle. In your girlfriend's case, the delay of nearly 10 days, combined with recent events such as hospitalization and antibiotic treatment, raises several important considerations.

Firstly, it’s essential to understand that menstrual cycles can be influenced by a variety of factors. Stress, significant changes in routine, illness, and medications can all play a role in altering the regularity of menstrual cycles. In your girlfriend's situation, the hospitalization for acute pyelonephritis (APN) and the subsequent use of strong antibiotics could indeed be contributing factors to her delayed menstruation. Antibiotics can disrupt the natural balance of hormones and the gut microbiome, which may indirectly affect menstrual cycles. Additionally, the stress associated with being hospitalized and the physical toll of an illness can also lead to hormonal imbalances that may delay menstruation.

Moreover, while you mentioned that your girlfriend had sexual intercourse on January 31st with protection, it is important to note that no contraceptive method is 100% effective. Although the use of condoms significantly reduces the risk of pregnancy, it does not eliminate it entirely. If there is any doubt about the possibility of pregnancy, it may be prudent for her to take a home pregnancy test. These tests are generally reliable and can provide peace of mind.

In terms of the timeline, it is not uncommon for women to experience irregularities in their menstrual cycles, particularly during times of stress or after significant health events. The average menstrual cycle can range from 21 to 35 days, and variations can occur due to various factors. If your girlfriend's cycles have been regular prior to this incident, it may simply be a temporary disruption.

If her period does not arrive soon, or if she experiences any unusual symptoms such as severe pain, heavy bleeding, or other concerning signs, it would be advisable for her to consult a healthcare provider. A medical professional can conduct a thorough evaluation, including a physical examination and possibly blood tests, to assess hormone levels and rule out any underlying conditions that may be contributing to the delay.

In summary, while the delay in your girlfriend's menstruation is understandably concerning, it could be attributed to the recent illness, antibiotic treatment, and stress. Taking a pregnancy test can help clarify the situation, and if the delay continues or if there are any worrying symptoms, seeking medical advice is the best course of action. Regular monitoring of her menstrual cycle and being attentive to her overall health will also be beneficial in managing any future concerns.
