Redness and hard lumps in the breast?

Hello Doctor, I have been experiencing breast pain and lumps since I was 18 years old.
Initially, the lumps were movable, and my chest would sometimes turn completely red.
I am now 22 years old, and the lumps have become fixed but still painful.
They are about the size of a dime, with the outer area being softer and the core being harder and more painful.
Could this be mastitis? Would applying a warm compress help improve the condition? Thank you.

A Xiaojie, 20~29 year old female. Ask Date: 2016/02/11

Dr. Zhou Jiazhen reply Breast and Thyroid

Dear Ms.
A: 1.
It is rarely mastitis, but it cannot be ruled out.
2.
Generally, painful lumps are mostly benign cysts, and some may be fibroadenomas; however, changes in the skin can be more difficult to explain.
It is recommended to have an examination at a breast surgery clinic.

Breast pain and the presence of hard lumps can be concerning symptoms, especially for someone who has been experiencing them since the age of 18. Based on your description, it is important to consider several potential causes, including mastitis, fibrocystic breast changes, and other breast conditions.

Mastitis is an infection of the breast tissue that results in breast pain, swelling, warmth, and redness. It is most commonly seen in women who are breastfeeding, but it can occur in non-lactating women as well. The symptoms you describe—pain, a hard lump, and redness—could suggest mastitis, especially if there is a history of infection or inflammation in the area. However, the fact that the lump has become immobile and remains painful raises the possibility of other conditions.

Fibrocystic breast changes are quite common and can cause lumps and breast pain. These changes are often related to hormonal fluctuations during the menstrual cycle. The lumps associated with fibrocystic changes can feel like soft or hard areas in the breast and may change in size and tenderness throughout the cycle. The description of your lump being soft on the outside and hard in the center could align with this condition.
It's also important to consider the possibility of a breast cyst or a benign tumor, such as a fibroadenoma. Cysts can be fluid-filled sacs that may feel hard or soft, and they can cause discomfort. Fibroadenomas are solid, non-cancerous lumps that can also be painful and may change in size.

Given that you have experienced these symptoms for several years, it is crucial to seek a thorough evaluation from a healthcare professional, preferably a breast specialist. They may recommend imaging studies, such as a mammogram or ultrasound, to better understand the nature of the lump and to rule out any serious conditions, including breast cancer.

As for your question about heat therapy, applying a warm compress to the affected area can sometimes provide relief from pain and discomfort. Heat can help improve blood flow and may assist in reducing inflammation. However, if you suspect an infection, it is essential to consult a healthcare provider before relying solely on home remedies, as antibiotics may be necessary to treat mastitis or any underlying infection.

In summary, while your symptoms could suggest mastitis, they may also be indicative of other benign breast conditions. It is essential to have a professional evaluation to determine the exact cause of your symptoms and to receive appropriate treatment. Regular monitoring and follow-up with your healthcare provider will be crucial in managing your breast health.
