Hard lump at the base of the penis?
Hello doctor! I have developed a raised bump resembling a pimple on the right base of my penis, and there is a white spot at the top of this red swollen area.
When I squeeze it, I can feel a hard lump and slight pain.
I initially thought it was a pimple, so I applied steroids, but I have not seen any improvement (the swelling has not gone down).
What could this be???

Dr. Wu Yuxin reply Dermatology
1.
Without actually seeing the lesion, it cannot be confirmed.
If it is very concerning, it is recommended to seek medical attention as soon as possible.
2.
Acne should not be treated with corticosteroid ointments.

More Info
When it comes to hard lumps at the base of the penis, there are several potential causes that could explain your symptoms. The description of a hard lump resembling a pimple, accompanied by redness, a white tip, and mild tenderness, suggests a few possibilities.
1. Pearly Penile Papules: These are small, harmless bumps that can appear around the corona of the glans penis. They are typically skin-colored or white and are not painful. However, they usually do not present as red or inflamed.
2. Folliculitis: This condition occurs when hair follicles become inflamed, often due to infection or irritation. It can present as red bumps that may be tender to the touch. If you have been using topical steroids, it may have exacerbated the condition if it was an infection.
3. Sebaceous Cysts: These are non-cancerous lumps that can occur when sebaceous glands become blocked. They can feel firm and may become inflamed or infected, leading to redness and tenderness.
4. Genital Warts: Caused by the human papillomavirus (HPV), these can appear as small, flesh-colored or white bumps. They can be tender and may have a cauliflower-like appearance.
5. Herpes Simplex Virus (HSV): If the lump is accompanied by other symptoms such as blisters or sores, it could be a sign of genital herpes. This viral infection can cause painful lesions.
6. Penile Cancer: Although less common, any persistent lump or sore that does not heal should be evaluated by a healthcare professional to rule out malignancy.
Given that you have applied topical steroids without improvement, it is essential to seek medical attention. A healthcare provider can perform a physical examination and may recommend further tests, such as a biopsy, to determine the exact nature of the lump.
Treatment Options
The treatment will depend on the underlying cause of the lump:
- For Folliculitis: Warm compresses and topical antibiotics may help. In some cases, oral antibiotics may be necessary if the infection is more severe.
- For Sebaceous Cysts: If they are not causing discomfort, they may not require treatment. However, if they become infected or bothersome, a healthcare provider can drain or remove them.
- For Genital Warts: Treatments include topical medications, cryotherapy (freezing), or surgical removal.
- For Herpes: Antiviral medications can help manage outbreaks and reduce symptoms.
- For Pearly Penile Papules: No treatment is necessary as they are benign, but if they are bothersome, laser therapy or other cosmetic procedures can be considered.
- For Penile Cancer: Early detection is crucial, and treatment may involve surgery, radiation, or chemotherapy depending on the stage of the cancer.
Conclusion
In summary, while there are several potential causes for the hard lump at the base of your penis, it is crucial to consult with a healthcare professional for an accurate diagnosis and appropriate treatment. Self-diagnosing and treating with topical steroids may not be effective and could potentially worsen the condition. Early evaluation and intervention are key to managing any underlying issues effectively.
