Abnormal secretion?

Hello, doctor! Last month, I had sexual intercourse with my boyfriend, but shortly after, I noticed some bleeding, so we stopped immediately.
However, I didn't experience any pain, and it wasn't my menstrual period.
This month, I had sexual intercourse twice without any bleeding, but then I bled again once.
The next day, I had a small amount of brownish-red discharge that required a panty liner, but it stopped at night.
This unusual discharge lasted for five days during the day, but not at night.
What should I do? I often stay up late; could hormonal imbalance cause this?

Miss F, 20~29 year old female. Ask Date: 2016/02/17

Dr. Lin Manying reply Obstetrics and Gynecology

Bleeding after sexual intercourse is a concern, and it is advisable to undergo a Pap smear for further evaluation.

Experiencing abnormal discharge, especially after sexual activity, can be concerning and may indicate various underlying issues. In your case, the occurrence of bleeding after intercourse, followed by abnormal discharge, warrants careful consideration.

Firstly, it's important to understand that bleeding after sexual intercourse, known as postcoital bleeding, can have several causes. Some common reasons include:
1. Cervical Irritation or Injury: This can occur if there is friction during intercourse, especially if there is insufficient lubrication. It can also happen if there are any lesions or infections present on the cervix.

2. Infections: Sexually transmitted infections (STIs) such as chlamydia or gonorrhea can lead to inflammation and bleeding. Other infections, such as bacterial vaginosis or yeast infections, can also cause abnormal discharge and irritation.

3. Hormonal Imbalances: Hormonal fluctuations can lead to changes in the menstrual cycle and may cause spotting or abnormal discharge. Factors such as stress, significant weight changes, or irregular sleep patterns (like staying up late) can contribute to hormonal imbalances.

4. Polyps or Fibroids: These benign growths in the uterus or cervix can cause bleeding and may lead to abnormal discharge.

5. Pregnancy-related Issues: If there is a possibility of pregnancy, bleeding could be related to implantation bleeding or other complications.

Given that you experienced bleeding after sexual intercourse on two occasions, along with abnormal discharge, it is advisable to seek medical attention. Here are some steps you can take:
- Schedule an Appointment: Visit a healthcare provider, preferably a gynecologist, to discuss your symptoms. They may perform a pelvic exam and possibly order tests such as a Pap smear, STI screening, or ultrasound to determine the cause of your symptoms.

- Monitor Your Symptoms: Keep track of your menstrual cycle, any additional symptoms (such as pain, changes in discharge color or odor), and the timing of the bleeding. This information can be helpful for your healthcare provider.

- Lifestyle Adjustments: Consider making lifestyle changes that can help regulate your hormones, such as improving your sleep hygiene, managing stress, and maintaining a balanced diet.

- Avoid Irritants: Until you see a doctor, avoid using any vaginal products (like douches or scented soaps) that could further irritate the area.

In conclusion, while occasional abnormal discharge or spotting can occur for benign reasons, the recurrence of these symptoms, especially after sexual activity, should not be ignored. It is essential to consult with a healthcare professional to rule out any serious conditions and to receive appropriate care. Your health and well-being are paramount, and addressing these symptoms early can lead to better outcomes.
