Testicular issues
Hello doctor, I recently noticed that my right testicle is upright while my left one is lying down.
Is this normal? There is no pain at all.

Xiao Wang: Hello, if there is no pain, it is likely not testicular torsion.
If you are still concerned, I recommend seeking medical attention.

It’s quite common for individuals to notice differences in the position of their testicles, and what you’re experiencing is generally considered normal. The testicles are not always symmetrically positioned; in fact, it’s typical for one testicle to hang lower than the other. This asymmetry is due to several factors, including anatomical differences, the presence of the spermatic cord, and the way the body regulates temperature for optimal sperm production.
In your case, the right testicle being upright while the left one is lying down is not inherently a cause for concern, especially since you are not experiencing any pain. The scrotum, which houses the testicles, is designed to allow for some movement and positioning variations. The muscles in the scrotum, known as the cremaster muscles, can contract or relax to raise or lower the testicles in response to temperature changes or physical activity. This mechanism helps maintain an optimal temperature for sperm production, which is typically a few degrees cooler than the body’s core temperature.
However, while variations in testicular position are usually normal, it’s important to be aware of any accompanying symptoms that could indicate a problem. If you were to experience pain, swelling, or any noticeable changes in the size or shape of the testicles, it would be advisable to consult a healthcare professional. Conditions such as testicular torsion, epididymitis, or varicocele can cause discomfort and may require medical attention.
In summary, the position of your testicles, with one being upright and the other lying down, is likely normal and not a cause for concern, especially in the absence of pain. However, if you have any doubts or if your situation changes, seeking advice from a healthcare provider is always a good practice. Regular self-examinations can also help you become more familiar with your anatomy and detect any unusual changes early on.
