What would you like to know about cervical cancer screening?
Hello Dr.
Lu: I heard that there is a type of Pap smear for human papillomavirus (HPV) that can detect the potential risk of cervical cancer early.
How accurate is the predictive rate? Is this screening available in Taoyuan? What are the costs? Please respond.
Mei Nu Hua, 30~39 year old female. Ask Date: 2002/07/29
Dr. Lü Lizheng reply Obstetrics and Gynecology
There are many types of human papillomavirus (HPV), among which types 16, 18, and 31 can invade cervical cells and induce cellular changes.
Some of these cellular changes may eventually progress to cancerous cells.
Therefore, detecting human papillomavirus can help us identify "high-risk" patients who need to be particularly cautious.
However, among these individuals, only a small number will actually develop cervical cancer in the future, so there is currently no need for excessive concern; regular monitoring is sufficient.
As of now, this type of screening has not yet been introduced in Taiwan.

More Info
Human Papillomavirus (HPV) screening is an essential component in the prevention and early detection of cervical cancer. The relationship between HPV and cervical cancer is well-established, as certain high-risk types of HPV, particularly types 16 and 18, are responsible for the majority of cervical cancer cases.
1. Effectiveness of HPV Screening: The effectiveness of HPV screening in predicting cervical cancer is high. Studies have shown that HPV testing is more sensitive than traditional Pap smear tests in detecting precancerous lesions and cervical cancer. When used in conjunction with Pap smears, HPV testing can significantly improve the detection rates of cervical abnormalities. The combination of these tests allows for better risk stratification; women who test positive for high-risk HPV types are at a higher risk for developing cervical cancer and may require closer monitoring or intervention.
2. Availability in Taoyuan: As for the availability of HPV screening in Taoyuan, it is essential to consult local healthcare providers or clinics. While some private clinics may offer HPV testing as part of their services, it may not be universally available in all public healthcare facilities. It is advisable to check with specific clinics or hospitals in Taoyuan to confirm whether they provide HPV testing and what the associated costs are.
3. Cost Considerations: The cost of HPV screening can vary widely depending on the healthcare facility and whether the test is covered by insurance. In Taiwan, the National Health Insurance (NHI) program covers Pap smear tests for women over 30 years old, but HPV testing may not be included in this coverage. If HPV testing is not covered, patients may need to pay out-of-pocket, which can range from a few thousand to several thousand New Taiwan Dollars. It is recommended to inquire about the costs directly with the healthcare provider.
4. Importance of Regular Screening: Regular screening for cervical cancer, including Pap smears and HPV testing, is crucial for early detection and prevention. Women are encouraged to start screening at age 21 and continue until age 65, with the frequency of testing depending on age and previous test results. For women aged 30 and older, co-testing (Pap smear plus HPV testing) is recommended every five years or Pap testing alone every three years.
5. Understanding HPV and Sexual Behavior: It is important to note that HPV is a sexually transmitted infection, and while condoms can reduce the risk of transmission, they do not provide complete protection. Therefore, regular screening is vital, even for those who practice safe sex. If a woman tests positive for high-risk HPV, it does not necessarily mean she will develop cervical cancer; most HPV infections resolve on their own. However, those with persistent infections should be monitored closely.
In conclusion, HPV screening is a highly effective tool for predicting cervical cancer risk, and while it may not be universally available in Taoyuan, it is worth exploring local options. Regular screening is essential for early detection and prevention of cervical cancer, and understanding the costs and availability of these tests can help women make informed decisions about their health. If you have further questions or concerns, it is advisable to consult with a healthcare provider who can provide personalized guidance based on your medical history and risk factors.
