Missed menstruation and urinary incontinence?
Hello, my menstrual cycle has been irregular; I haven't had my period for three months.
In the past, I have also experienced missed periods and consulted a doctor.
After taking medication, my period returned but only slightly.
What can I do to improve this? Should I continue to see a gynecologist? Additionally, I have recently experienced urinary leakage.
Although I haven't engaged in any vigorous exercise, I notice a drop or two of urine leaking when I walk or stand.
Should I consult a gynecologist for this, or is it better to see another specialist? Thank you.

Dr. Cao Guotao reply Obstetrics and Gynecology
Hello, it is recommended to visit a hospital for a gynecological examination and blood tests.
At your age, it is uncommon to have issues with urinary incontinence, but it would be good to check together.
Wishing you good health.

Menstrual irregularities and urinary leakage are two distinct but potentially interconnected issues that can significantly impact a woman's quality of life. Given your symptoms of missed periods and urinary leakage, it is essential to approach these concerns systematically.
Firstly, regarding your menstrual irregularities, it is not uncommon for women to experience fluctuations in their menstrual cycles due to various factors, including hormonal imbalances, stress, weight changes, or underlying medical conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders. Since you mentioned that you have previously consulted a doctor and received medication that resulted in a minimal menstrual flow, it would be advisable to return to your gynecologist for further evaluation. Your doctor may recommend additional tests, such as blood tests to check hormone levels, an ultrasound to assess the ovaries and uterus, or other diagnostic procedures to determine the underlying cause of your irregular periods. It is crucial to address this issue, as prolonged menstrual irregularities can lead to complications such as anemia or fertility issues.
In addition to menstrual irregularities, you also mentioned experiencing urinary leakage, which can occur for various reasons. The leakage of urine, especially when it happens during activities like walking or standing, may indicate a condition known as stress urinary incontinence. This condition is often related to weakened pelvic floor muscles, which can occur due to factors such as childbirth, aging, or hormonal changes. While it is possible to consult a gynecologist about urinary leakage, you may also benefit from seeing a urologist or a urogynecologist, who specializes in urinary disorders and pelvic floor issues. These specialists can provide a comprehensive evaluation and recommend appropriate treatment options, which may include pelvic floor exercises (Kegel exercises), lifestyle modifications, medications, or surgical interventions if necessary.
It is essential to address both issues concurrently, as they may be interrelated. Hormonal imbalances affecting your menstrual cycle could also influence your urinary function. For example, estrogen plays a role in maintaining the health of the pelvic floor muscles and urinary tract. Therefore, a holistic approach that considers both your menstrual health and urinary function is recommended.
In summary, I recommend the following steps:
1. Return to your gynecologist: Schedule an appointment to discuss your menstrual irregularities in detail. Be prepared to provide information about your menstrual history, any medications you have taken, and any other symptoms you may be experiencing.
2. Consider seeing a urologist or urogynecologist: If your urinary leakage persists, it would be beneficial to consult a specialist who can provide targeted treatment options for urinary incontinence.
3. Lifestyle modifications: In the meantime, consider lifestyle changes that may help improve both your menstrual and urinary health. This includes maintaining a healthy diet, staying hydrated, managing stress, and engaging in regular physical activity.
4. Pelvic floor exercises: If you are diagnosed with urinary incontinence, pelvic floor exercises can strengthen the muscles that support the bladder and may help reduce leakage.
5. Follow-up: Keep track of your symptoms and follow up with your healthcare providers as needed to ensure that both your menstrual and urinary issues are being effectively managed.
Taking these steps will help you gain a better understanding of your health concerns and lead you toward appropriate treatment options. Remember, seeking help is a positive step toward improving your overall well-being.
